• Stuart's Opera House After-School Music Program (AMP) 2025-2026 School Year

    Application for New Students
  • Welcome to AMP 2025-2026! For more info about AMP, please visit our website. We recommend having a good sense of the program before applying so students and families know what to expect.

Note: Applicatons were due by August 1, 2025. We already have a surplus of applicants, but you are still welcome to apply to be put on the waiting list.

AMP runs Mondays, Tuesdays, Wednesdays, and Thursdays. The program will start on Monday, September 8 and will run through the 2025-2026 school year roughly following the same calendars as the local school districts. Students commit to either Monday, Tuesday, Wednesday, or Thursday and attend weekly through the school year. The programs run from 3:30-6:00 pm. 3:30-4:00 pm is for student to arrive, check-in and have a snack. Instruction/rehearsals begin at 4:00 pm and go until 6:00 pm.

Please contact Assistant Director of Arts Education, Adam Remnant, at adamr@stuartsoperahouse.org with any questions or concerns regarding the program. We are also reachable by phone at 740-753-1924. Thank you!

  • Student Information

  • Caregiver Info

The program uses email and text messaging to relay info to students and families on a regular basis. Most communication is done via email, but we will use text messaging on occasion to let caregivers know about weather cancellations/delays or other short-term notifications. By default, we will use the caregiver info to send regular communication via email and text message.
  • STUDENT COMMUNICATION INFO (OPTIONAL)

Student often wish to also receive email and text updates to their own contact info. You are not required to provide this but can add their info below if they wish to receive updates.
  • Additional Communication Contact (Optional)

Note: this is an opportunity to provide contact info for any other family member / caregiver who should receive regular updates.
  • Emergency Contact Info

Please provide contacts that we can reach in an emergency.
  • Safety Information

  • Scheduling / Forming Bands

We do our best to place each student in a band that fits their skill level, age range, and musical interest. We do our best to accommodate placing students together with friends they wish to be in a band with. That said, there are a lot of variables to make it all work. Please answer the following questions to help us find the best fit for each student.
  • Audio Production (Optional)

While we used to have a dedicated day for Audio Production, we are planning to integrate Audio Production into our regular AMP program this school year. However, if students are interested in Audio Production and would like to get more hands-on experience, we do offer the opportunity for students to attend AMP as an Audio Student to learn and help engineer audio recording with our AMP students. Audio students will engage in pre-production, recording, beat-making, mixing, and mastering student + guest recordings. Students can join just as an audio student or if they are a regular AMP student, they can attend as an audio student on a separate day.
  • Student Interests

  • Please indicate which instrument is their first choice and which is their second choice from the options below. The instruments we offer are guitar, vocals, bass, drums, and keyboard. (Students can attend the program playing an instrument we don't offer, but our instruction abilities may be limited and students may have to provide their own instrument.)

  • Code of Conduct Agreement

All students must read and sign the Code of Conduct agreement.
  • In order to maintain a safe environment, everyone who attends Stuart’s Opera House’s education programs must read and sign the Code of Conduct Agreement. Thank you for your cooperation.

I understand that Stuart’s Activities may pose risks of bodily injury due to the inherent nature of each activity. I fully understand the risks and dangers associated with participation in Stuart’s Activities, that the risks and dangers may be caused by my own actions, or inactions, the actions or inactions of others participating in Stuart’s Activities, or the condition in which the Stuart’s Activities take place. I also fully understand that there may be other risks and social and economic losses either not known to me or readily foreseeable at this time, and I FULLY ACCEPT AND ASSUME ALL SUCH RISKS AND ALL RESPONSIBILITY FOR LOSSES, COSTS, AND DAMAGES I incur as a result of my participation in Stuart’s Activities and assume all risks and responsibilities surrounding my participation in these activities.

    2. I agree and warrant that I will examine and inspect each Stuart’s Activity in which I take part, and if I observe any conditions that I consider to be unacceptably hazardous or dangerous, I will notify the proper authority in charge of the Stuart’s Activity and will refuse to take part in the Stuart’s Activity until the condition has been corrected to my satisfaction.

I understand that this Release of Liability/Indemnity/Emergency Treatment form binds my family, heirs, executors, administrators, and assigns, as well as myself.

    4. In case of emergency, accident, illness, or other incapacity occurring while under the Stuart’s authority, I give my permission to be treated by medical professionals and admitted to the hospital if necessary. This authorization applies whether or not the charges are covered by my insurance, and I am responsible for all reasonable medical and emergency expenses incurred on my behalf, regardless of whether I would have authorized such expenses under separate circumstances.

    5. I agree that I will follow all Stuart’s rules and will remain within the parameter of Stuart’s Activities at all times.

    6. I have read this agreement, fully understand its terms, understand that I have given up substantial rights by signing it, and have signed it freely and without any inducement or assurance of any nature, intend it to be a complete and unconditional release of all liability to the greatest extent allowed by law, and agree that if any portion of this agreement is held to be invalid, the balance notwithstanding, shall continue in full force and effect.

  • ADDITION FOR MINORS / IN THE PROGRAM:

  • Media Release Form

If the student is a minor (under 18), their adult guardian is responsible for reading and signing the Media Release Form. If the student is over 18, they are responsible for reading and signing the Media Release Form.
  • In this case, "Media" refers to all Stuart’s Opera House Arts Education Program Footage, Videos/Audio Recording, Promotional Pieces, Documentaries, DVD, Photographs, etc.

Stuart’s Opera House shall own all rights, title(s), and interest(s) in and to the program(s), including the recording(s), to be used and disposed of at the discretion of Stuart’s Opera House.

  • ADDITION FOR MINORS IN THE PROGRAM:

  • For Caregivers: Donations

  • We strive to make AMP completely free for families to eliminate financial barriers to arts education. This is still the case in the 2024-2025 school year.

However, due to rising costs of the program, we are adding a suggested annual donation of $200 to support the program. This is slightly less than half of what we spend on each student each year ($550) when accounting for instructor pay, equipment and snacks (not accounting for use of the facility or salaried staff pay).

If your student is accepted into the AMP program, we will email you a link to make a donation to support the program. This is not required, so please still submit your application even if it's not in your budget currently. We will also allow an option to donate a different amount than the suggested $200 donation in order to meet your budgetary needs. Additionally, you can choose to make payments in semester installments if that works better for you.

  • WHAT HAPPENS TO MY APPLICATION?

We do our best to accommodate as many students as possible, however, there is the chance that we could receive more applicants than we have capacity to accept. Returning students always take priority but we have new spots open every year. We do our best to comb through every application to figure out bands and schedules that will work for everyone. We will follow up with applicants no later than Friday, August 28 to inform you of your acceptance into the program. We might reach out to you about schedule or instrument flexibility between now and then. If we are unable to accept a student this year, they will be put on a waiting list. We do occassionally have students drop out of the program and have spots open up. Thank you for supporting Arts Education at Stuart's Opera House!
