Important Info, please read:

1. Please show up on time to your appointment. I recommend coming in 5-10 minutes earlier to pick your designs and fill out paperwork. There will be many people scheduled for this event and one person running behind can affect everyone's schedule.

2. Flash days can be unpredictable and there may be some amount of waiting involved. We appreciate your patience and understanding!

3. Please be certain of the number of tattoos (2 max) you want before submitting this form. NOTE: If you are only scheduled in for 1 tattoo, we will only allot time for 1 tattoo for your appointment. If you are scheduled in for 2 tattoos, you will be getting a longer appointment time and be responsible for the price of 2 tattoos.

4. The price is $100 for black & grey and $130 for color. 2-3 inches in size. ARMS AND LEGS PLACEMENT ONLY.

5. I will only be tattooing from the flash posted for this event. The only permitted changes are to remove certain parts of the design (e.g. taking out the 13, text, or sparkles), but no additions to the design can be made.

6. Please be prepared to leave a nonrefundable deposit of $30 to secure your appointment. The deposit will be taken off of the total and is not an additional cost. The rest will be paid on the day of the appointment in cash.

7. If for any reason you are unable to attend, please let us know as soon as possible so that we can get someone off the waitlist.
