Acknowledgment of Volunteer Role and Responsibilities
I, the undersigned volunteer, understand that I am offering my services to Second Wind Philly and its associated initiatives on a voluntary basis, without expectation of compensation. I agree to perform my duties to the best of my ability and conduct myself with compassion, professionalism, and respect toward all individuals served by this organization.

I understand that my responsibilities may include, but are not limited to:

Distributing food, clothing, and hygiene items
Assisting with outreach to individuals experiencing homelessness, addiction, and mental illness
Helping connect individuals with resources for detox, treatment, mental health, housing, and veteran support
Participating in events or initiatives hosted by Second Wind Philly and/or the Kensington Rising Initiative
Health and Safety Waiver
I acknowledge that participating in outreach activities may expose me to risks including, but not limited to: injury, illness (including infectious diseases), emotional distress, or environmental hazards. I understand and agree that:

I am volunteering at my own risk.
I will follow safety guidelines and training provided by Second Wind Philly staff or coordinators.
I am physically and emotionally able to carry out my volunteer duties.
Release of Liability
I, for myself and on behalf of my heirs, assigns, and personal representatives, hereby release, indemnify, and hold harmless Second Wind Philly, its staff, directors, partners, coordinators, volunteers, and affiliated organizations from any and all liability, claims, or demands of whatever kind or nature, either in law or in equity, which arise or may hereafter arise from my participation in volunteer activities with Second Wind Philly.
Consent to Use of Image and Testimonials
I grant permission for Second Wind Philly to take photographs, videos, and written or oral testimonials of me during volunteer activities. I understand that these materials may be used for outreach, promotional, and fundraising purposes on social media, websites, print materials, or grant applications, and I consent to such use without expectation of compensation.

I agree to maintain the confidentiality of all sensitive information I may encounter during my volunteer work, including but not limited to names, medical conditions, addiction or mental health status, or personal stories of the individuals served.
