If you answered "yes" to either question above, please detail further below. Please note: Emergency medications/epi-pens/etc must be dropped off to the camp admin/office upon arrival. OFC staff will not administer non-emergency medication during camp, a parent/guardian must return to administer.

- Treat everyone with respect and consideration while you are at OFC and follow directions.
- Keep your language, behavior, and dress appropriate.
- We do not tolerate discrimination of any kind and have 0 tolerance bully policy.
- We do not tolerate any drinking, smoking, drugs, or PDA at OFC events

- By my signature below, I hereby grant OFC Creations permission to use my camper’s likeness in a photograph, video, or other digital media (“photo and video”) in any and all of its publications, including web-based and print publications, without payment or other consideration. I understand and agree that all photos will become the property of OFC Creations and will not be returned.
- By my signature below, I recognize and understand the risks involved in participating in the program and willingly and voluntarily accept these risks. I hereby surrender any right to seek reimbursement from OFC Creations and its owners, employees, volunteers and other agents for injury sustained and liability incurred during my camper’s participation in the activity described above. By my signature, I warrant that I am not relying on any oral representations, statements or inducement apart from the statements made on this form. I understand that OFC Creations will not be held responsible for virus infection on site, and I agree to hold harmless and waive OFC Creations of any liability.
