Madison Elite Track Club Athlete and Adult Volunteer Registration

PLEASE READ CAREFULLY 

Thank you for your interest in the Madison Elite Track Club of Dane County.  Please realize Adults are not charged, but must participate in some way.  Membership fee includes practices, t-shirt, and membership with USA Track & Field and AAU and track meets.

We will register all athletes with USA Track & Field and AAU.  To do this we will need a copy of your child’s birth certificate.

 By Signing Up Below I/We understand:

- There is a time commitment to you, the team and teammates by participating. 

- Most practices and games are mandatory.  Missing practices and games will affect playing time.

- I/We the parent(s) of a athlete for the Madison Elite Track club hereby give my/our child approval to his participation in any/all activities during the season.

- I/We assume all risks and hazards incidental to such participation including transportation to and from activities, and I/We do herby waive, absolve, indemnify and agree to hold harmless the Puma organization, participants and persons transporting my/our child – except to the extent and in the amount covered by liability insurance.

- I/We understand Medical insurance will NOT be provided by the Madison Elite Track Club.

- I/We will furnish a certified birth certificate.

 

Payment of all fees are due with the submission of this registration.

DO NOT E-SIGN UNTIL YOU HAVE READ THE ABOVE STATEMENT.

I authorize the use of photographs or videos that include my image and the image of my child or children for promotional, informational or other reasons deemed to be in the best interest of Madison Elite Track Club.

The Parent/Guardian of the participant acknowledges that there is a degree of risk and harm involved in the sport of track and field. By signing this document, the parent/guardian accepts sole risk and responsibility for the participant and agrees to hold harmless the Madison Elite Track Club, its administrators and coaches in the event that physical harm or death occurs to the participant during any track event, practice, or team travel.

In the event of an emergency, I give the administrators of the Madison Elite Track Club permission to provide my child with emergency medical care.

Athlete Health Information

If you check yes to any health conditions, you must have your child’s physician sign this form indicating that your child’s participation in track and field will not be harmful to his or her health.
