  • ⚠️ Important to know

The more questions you tick 'yes,' the more likely it is your child shows traits often seen in ADHD. But this is not a diagnosis. It’s a reflection tool to highlight ADHD presentations that don’t always fit the 'typical' picture.

You can fill this checklist out for your child (especially if they’re younger), or — if they’re old enough — they can complete it themselves. Sometimes the most useful insights come when you look at both versions side by side.

This tool cannot confirm or rule out ADHD. Only a qualified professional can do that, through a full clinical evaluation.

If your answers bring up concerns or feel uncomfortably accurate, consider taking them to your GP, psychologist, or psychiatrist. Think of this as a way to start the conversation, not finish it.

  • I’ve been told things like: “You drift off into your own world,” or “You’d do really well if you just focused more,” or “You don’t work to your full potential.”*
  • I get embarrassed at school when I miss what the teacher said — either I wasn’t listening, I forgot straight away, or I just didn’t get it the first time they said it.*
  • I hate being put on the spot in class, because sometimes I’ve missed what was said and what the question was.*
  • I blurt out answers to questions before they are fully asked at school, or forget to put up my hand or I tend to interrupt others.*
  • I get stomachaches or headaches that are unexplained.*
  • I get told I never stop talking — I always have something to say, even if there’s nothing much going on.*
  • Even if people remind me, I still forget what I’m meant to do next, or take with me when I’m getting ready for school or extra curricular activities etc.*
  • It’s hard for me to be patient when I have to wait my turn.*
  • I feel like I worry, or have my feelings hurt*
  • I lose things like homework or personal belongings more than other kids I know.*
  • Sometimes people tell me to stop fidgeting or moving*
  • Sometimes I start something and don’t finish it before I start something else. I find half finished projects I've started around the house*
  • Sometimes I make mistakes in my schoolwork because I rush to get it done or I miss details.*
  • Sometimes I feel left out because other kids don’t like me, but I don’t understand the reason.*
  • Sometimes people say I’m too intense or 'in their face,' OR I don’t realise when other kids don’t want to play my game so I 'take over'.*
