I AGREE to pay a meter deposit in the amount of $75.00 and a Service Fee in the amount of $25.00 to have the water turned on at the above location (water/sewer). I AGREE to pay a deposit in the amount of $150.00 to have my services connected through the City of Oakdale (sewer ONLY). I WILL pay the water and sewer bill each month until I sign a release to have services discontinued. If I shouldn't pay my bill, it will be placed in collections with a 40% interest rate.

WATER CONNECTION POLICY

1. City Worker will be at the above address between the hours of 3 PM & 4:30 PM.
2. Someone with a VALID Driver's License MUST be home during CUT ON.
3. Water WILL NOT be turned on if no one is at the above address. 4. If no one is at the above address, CUT ON will be rescheduled for next business day.
5. Be prepared to sign tick before City Worker leaves.
6. Additional service call for CUT ON will have a $50.00 Service Charge.
7. Same day reschedule AFTER 4:30 PM is $75.00.
