Iunderstand that, during the course of my participation in James R. Taylor and Sons, Inc. "Military On Top", and those acting with the permission or authority of James R. Taylor and Sons, Inc., may capture my name, likeness, image, or voice in photographic, audio, video, digital or other forms ("Media" I recognized that all Media - including film, photographic prints, audio, video or digital files, including those photos submitted with my nomination - become the exclusive property of James R. Taylor and Sons, Inc. In addition, I hereby permit James R. Taylor and Sons, Inc. and those acting with James R. Taylor and Sons, Inc. permission or authority, to use my name and Media in materials available to existing and potential customers, staff and individuals within the community, including in publicity or promotional materials for James R. Taylor and Sons, Inc. I hereby waive any right to inspect or approve: (a) the finished Media, (b) any printed matter that may be used in conjunction with the Media, or (c) the eventual use to which the Media may be applied. I waive all right to compensation, royal- ties and damages, including all claims in tort, defamation, right to privacy or right of publicity claims. I sign this authorization and release on behalf of myself and all of my minor children who may be participating in "James R. Taylor and Sons, Inc. "Military On Top". This agreement constitutes the sole, complete, and exclusive agreement regarding the Media, and I am not relying on any other representation, whether oral or written.

Please provide a photo of the current roof. Nominations need to be sent by mail to P.O. Box 163, East Troy, WI 53120. Nominations must be postmarked by April 1, of the current year to be considered as the current year recipient.

"Military On Top" is not a contest or lottery. One deserving nominee each year will be selected to receive 1 new roof solely gratis at the discretion of James R. Taylor and Sons, Inc. Nomination Rules for "Military on Top": 1. A nominee must be a legal resident of the State of Wisconsin and Walworth County who is 18 years of age or older, who is currently serving or has previously served in the United States military (Army, Navy, Air Force, Marines, National Guard, Reserves Proof of a current military ID or a DD form 214 must be submitted. Entries must be submitted to James R. Taylor and Sons, Inc. at 2115 Division Street, East Troy, WI 53120. James R Taylor and Sons, Inc. (hereinafter JRT & Sons, Inc reserves the right to examine identification and may reasonably choose to accept or deny awarding the roof based on the identification presented. 2. Nominee may only receive one complete roof system installed according to local and manufacturer specifications. JRT & Sons, Inc. determine the asphalt shingle and manufacturer. Recipient determines or chooses color. 3. Nominee must own the home and be in good standing on their monthly mortgage payments. 4. Employees (including, without limitation, part-time or temporary employees) of JRT & Sons, Inc., and their immediate family (i.e., current and ex- spouses and domestic partners, parents, grandparents, children, grandchildren, and siblings and in-laws and steps of any of the foregoing categories) and other household members (i.e., roommates, housemates, significant others, and partners of each legally residing at the same address) of each of the above are NOT eligible to enter and/or to receive the free roof.

Recipient chosen hereby agree to strictly follow all instructions and directions given by JRT & Sons, Inc. and staff at any event related to JRT & Sons, Inc. "Military On Top", and understand that any intentional failure to comply with the JRT & Sons, Inc. instruction, process, procedures or rules at the event may result in immediate disqualification (in JRT & Sons, Inc. sole discretion Any decision by JRT & Sons, Inc. will be final and without question. JRT & Sons, Inc. may elect to notify the winner by telephone or send one written notification letter by first class mail. 8.In no event will any "deal-making" between nominees be permitted. JRT & Sons, Inc. reserves the right to disqualify any person that it suspects has participated in or offered or accepted any type of "deal-making" with respect to any JRT & Sons, Inc. giveaway with other nominees or third parties. All required paperwork (including, without limitation, liability release agreements) has been completed by the pending recipient within the required deadlines. 9. Notwithstanding anything to the contrary herein or stated JRT & Sons, Inc. social media or adverting, no person will be entitled to receive any type of roof until after their eligibility has been confirmed or accepted by JRT & Sons, Inc. and all required paperwork (including, without limitation, liability release agreements) have been completed by the pending recipient within the required deadlines. 10. JRT & Sons, Inc. "Military On Top", reserves the right to substitute an award of equal or greater value for JRT & Sons, Inc. "Military On Top". Non-cash awards are not redeemable for cash. No transfer or assignment of awards is allowed, except as may be determined by JRT & Sons, Inc. "Military On Top", in its sole and absolute discretion on a case-by-case basis. 11. Recipient is prohibited from transferring any part of "Military On Top Award" without JRT & Sons, Inc. authorization (which authorization must be in writing by a JRT & Sons, Inc. representative and shall be granted or withheld in JRT & Sons, Inc. sole discretion on a case-by- case basis 12. Winners are responsible for paying all applicable local, county, state and federal taxes on award based on the estimated retail value of the award, as set forth in the nomination rules, and will be issued an IRS 1099 form for all awards won from JRT & Sons, Inc. where the aggregate value of all awards is $600 or more. 13. The choice of products offered may be limited by JRT & Sons, Inc., or certain specific products or manufacturers may be excluded. Recipient will receive more information on such restrictions and limitations after they have been selected.

1.Odds of being selected depend in part on the number of nominations received via mail to JRT & Sons, Inc. as well as other criteria. 2. JRT & Sons, Inc.'s management shall be the sole arbiter in all matters relating to the nominations and in the interpretation of nomination rules. Their decisions will be final. Nominations into "Military On Top" constitutes agreement by nominees to abide by these rules, as well as any other rules established by JRT & Sons, Inc. 3. If any Nominee is disqualified or any awarded material is forfeited in accordance with the applicable "Military On Top" rules, JRT & Sons, Inc. and Veteran Committee have no obligation whatsoever to notify any such Nominee of any such disqualification or forfeiture.

NOMINATED for "Military On Top" 2.Nomination may be made for oneself or on behalf of someone else provided the nominee meets the military service requirements. Incomplete or illegible entries may be disqualified in JRT & Sons, Inc. sole discretion. 3.All nominations become the property of JRT & Sons, Inc. and will not be acknowledged or returned. Nominations received that are mutilated, tampered with, illegible, or from ineligible entrants will be void. Nominations that are determined to be fraudulent will be void. Incomplete or illegible entries may be disqualified in JRT & Sons, Inc. sole discretion. 4. All nominations must be received by mail April 1 of the current year. Such entries will only be accepted by first class United States mail and not by fax, hand delivery, overnight or same day courier. Only one nomination is permitted per envelope (multiple entries within the same envelope will be disregarded Mail-in entries must be legible and contain all information and documents/materials required. 5. The recipient will be selected in accordance with the criteria set forth on the nomination form any announcement with respect to "Military On Top" and may include one or more of the following types of modes of nominating, without limitation: submitting essays, photos, or simply submitting the name and relevant information for the nominee. The submitted materials for nominations will be judged by the "Military On Top" Veteran committee, chosen by JRT & Sons, Inc. and the decision of the judges will be final as to all matters. 6. JRT & Sons, Inc., is requiring nominees to submit photographs of the roof in need (via mail) the following provisions will apply: (a) All entrants who submit a photo agree to the following (and JRT & Sons, Inc. may require, as a condition of entry, that entrants sign a separate agreement. See Media & Publicity release form on www.jrtaylorandsons.com Entrant represents and warrants that he/she own the rights to, or have obtained permission to use, the photo submitted with the entry. Entrant represents and warrants that he/she authorizes JRT & Sons, Inc. to post the photo on JRT & Sons, Inc. website or social network sites. (b) JRT & Sons, Inc. and their respective affiliates, and agents may use for advertising and/or promotional purposes, entrant's name, image, photo submission, other entry materials and/or biographical information on radio, TV broadcasts, newspaper advertisements, on websites and/or any other medium in connection with participation in the Military On Top program or otherwise without the payment of any additional compensation to the nominee. Nominee understands and agrees that JRT & Sons, Inc., shall be the sole and exclusive owner of the photograph submitted, whether or not the photo is selected as a winner, and entrant agrees to sign any documents requested by JRT & Sons, Inc. to evidence such transfer of ownership. By submitting a photo for "Military On Top", entrant agrees to indemnify James R Taylor and Sons, Inc. should any representations and warranties made by entrant herein or in the applicable nomination rules prove to be false or inaccurate. (c) All entries and photographs submitted become the property of James R Taylor and Sons, Inc. and will not be acknowledged or returned. (d) James R Taylor and Sons, Inc. and staff may review each photograph for unsuitable content, as determined in James R Taylor and Sons, Inc. sole discretion. All photographs containing any content JRT & Sons, Inc. deems unsuitable shall be disqualified in the sole discretion of JRT & Sons, Inc. No notification of such disqualification will be provided. DO NOT SEND ANY PHOTOS CONTAINING NUDITY, INDECENCY, ILLEGAL DRUG USE, OR OBSCENE GESTURES OR SITUATIONS. All of the foregoing

4. By participating in a JRT & Sons, Inc. "Military On Top", Nominee hereby agree and each Nominee hereby remises, releases and forever discharges and agrees to indemnify and hold harmless JRT & Sons, Inc., its affiliates, parents, assigns, successors, employees, sponsors, agents and all others connected with them and the promotion, contest, and/or event from any and all liability, claims, actions, and damages sustained or incurred by participation in the said promotion, contest, and/or event and the receipt and use of any prize(s) awarded (if any) through such promotion, contest, and/or event arising in any manner whatsoever, including but not limited to an act or omission, whether negligent, intentional or otherwise caused by JRT & Sons, Inc., its affiliates, parents, assigns, successors, employees, sponsors, agents and all others connected with them and the promotion, contest, and/or event. 5.By nominating, JRT & Sons, Inc. "Military On Top", entrants understand that JRT & Sons, Inc. staff may choose to comment on any entry. Entrants waive any right to make any claim against JRT & Sons, Inc. or veteran committee with respect to any comments - disparaging or otherwise - made regarding such entrant and/or his/her entry, entry materials, performance, voice, appearance, participation and/or any other information provided or disclosed to JRT & Sons, Inc. "Military On Top", during the course of JRT & Sons, Inc. "Military On Top". 6. By accepting the free roof in a JRT & Sons, Inc. "Military On Top" program, the recipient hereby consents to JRT & Sons, Inc. usage of any one or more of the following for any advertising and promotional purpose in any medium whatsoever without payment of any additional consideration: Recipient's name; voice; likeness; biographical information; his/her participation in the contest; and the substance of the recipient's entry form, essays, photos or any other contest submissions. Recipient understand agree that JRT & Sons, Inc. may take photographs and/or videotape and/or audiotape the recipient and/or the roof in connection with any JRT & Sons, Inc. "Military On Top", (before, during or after) and that JRT & Sons, Inc. will own any such photos, audio and/or video and shall be free to utilize such materials in any way it sees fit during or after JRT & Sons, Inc. "Military On Top", with no compensation to the Nominees, including without limitations, posting any such photographs, audio and/or videos on its website. 7. JRT & Sons, Inc. "Military On Top", is not responsible for any technical difficulties, natural disaster, act of God, or mis-delivered or un-delivered mail or any other factor that may prevent an individual from completing or JRT & Sons, Inc. from receiving (accurately or otherwise) the nomination or related materials for JRT & Sons, Inc. "Military On Top" program. 8. JRT & Sons, Inc. reserves the right to prohibit the participation of an individual from any or all JRT & Sons, Inc. "Military On Top", awards if fraud or tampering is suspected, or if the individual fails to comply with any requirement of participation as stated herein or with any provision in the applicable JRT & Sons, Inc. "Military On Top" rules. JRT & Sons, Inc. "Military On Top" program is subject to all applicable laws and regulations and is prohibited. 10. JRT & Sons, Inc., reserves the right to: (i) terminate or declare any nomination null and void and rescind any award, if in its sole judgment, the rules or the integrity of the program have been violated or compromised in any way, intentionally or unintentionally by any person whether or not a participant in the program; (ii) alter or amend these program rules at any time; and (iii) stop or conclude the JRT & Sons, Inc. "Military On Top", at any time without prior notice.

11.The selected nominee for a free roof will be notified May of the current year. Details of the work to be performed to install new roofing material will be issued at the time of notification. 12. These are general nomination rules, and rules for individual nominations may vary. To the extent that any specific nomination rules differ from these rules, the specific nomination rules will govern and control.

Acknowledgement of Terms & Conditions 1. Accuracy of Information: By submitting this form, the nominator certifies that all information provided is accurate and truthful to the best of their knowledge. 2.Consent to Share Information: The nominator consents to the use of submitted details for the purpose of evaluating the nomination and agrees that information may be shared with the selection committee. 3. Non-Guarantee of Selection: Submission of this form does not guarantee the nominee's selection, as all nominations are subject to review and evaluation. 4.Submission Deadline: Nominations submitted after the stated deadline will not be considered. 5. Retention of Submissions: All nomination submissions become the property of James R. Taylor & Sons Inc. and will not be returned unless explicitly requested. 6. Permission for Publicity: If the nominee is selected, the nominator and nominee grant permission for their names, stories, and photographs to be used in promotional materials and announcements. 7.Acknowledgment of Terms: By submitting this form, the nominator acknowledges that they have read, understood, and agreed to these terms and conditions. 8.Electronic Consent: Nominator agrees that electronic signatures or acceptance via email are as valid as original signatures for all purposes under this agreement.
