This form is for adults (18 years or older) seeking to receive the Sacrament of Confirmation, who are: (1) a Baptized Catholic, and (2) have received their first Sacrament of Communion.

If you are not a baptized Catholic and/or have not received the Sacrament of Communion, please instead complete the O.C.I.A. Inquiry Form by clicking here.

Completion of this form does not complete registration or enrollment into the program.
