1. Acknowledgment and Assumption of Risk
I, the undersigned parent or legal guardian of the above-named child, acknowledge that participation in activities at an indoor playground involves physical activity and inherent risks of injury, including but not limited to sprains, falls, collisions, and other accidents. I understand these risks and voluntarily allow my child to participate.
2. Release of Liability
In consideration of the opportunity for my child to participate in the event, I hereby release, waive, discharge, and hold harmless The Kidz Fun Zone, its owners, staff, event organizers, agents, affiliates, and volunteers from any and all claims, demands, causes of action, or liabilities for injuries, damages, or losses, including those caused by the negligence of the released parties, arising out of or in connection with my child's participation in the party and use of the playground facility.
3. Medical Treatment Authorization
In the event of an emergency, I authorize The Kidz Fun Zone staff to secure proper medical treatment for my child. I understand I will be notified immediately and I am responsible for any and all medical expenses incurred.
4. Photo and Video Release
I give permission to The Kidz Fun Zone to take and use photos or video of my child for promotional and marketing purposes, without compensation.
5. COVID-19 and Communicable Disease Disclaimer
I understand and accept the risk that participation may expose my child to communicable diseases including COVID-19. I voluntarily assume this risk and waive any liability for exposure-related issues.
6. General Provisions
This release shall be binding on me and my heirs, legal representatives, and assigns.
I confirm that I am the legal parent or guardian of the child named above and legally authorized to sign this waiver.
I have read and fully understand this release and sign it voluntarily.
