This Media Release and Consent Agreement (“Agreement”) is entered into by and between the undersigned individual (“Client”) and Bridge Performance LLC, a limited liability company organized under the laws of the State of New York.
1. Consent to Record and Use Media
The Client hereby grants Bridge Performance LLC, including its owners, employees, contractors, and agents, the irrevocable right and permission to photograph, video record, and/or audio record the Client during training sessions, consultations, or related activities conducted under Bridge Performance LLC.
The Client authorizes the use, reproduction, and publication of such media by Bridge Performance LLC, in whole or in part, for any lawful purpose, including but not limited to use in:
Social media posts (e.g., Instagram, TikTok, Facebook)
-Website content
-Marketing materials
-Educational resources
-Promotional advertisements
-This authorization extends to all languages, formats, and media now known or later developed.
2. No Compensation
The Client understands and agrees that they will not receive any compensation, monetary or otherwise, for the use of media as described in this Agreement. The Client waives any right to inspect or approve the finished product or media in which their likeness appears.
3. Ownership and Rights
All media content created or obtained by Bridge Performance LLC shall remain the sole and exclusive property of Bridge Performance LLC. The Client agrees that the Company has full ownership of any media created during training sessions and related activities.
4. Confidentiality and Discretion
Bridge Performance LLC agrees to use reasonable efforts to avoid disclosing personal health or private information not relevant to the media's intended purpose, unless explicitly authorized by the Client. No sensitive health or medical details will be disclosed in any public media without prior written consent.
5. Revocation
The Client may revoke this consent in writing at any time. However, such revocation will not apply to any materials already created, published, or in distribution prior to receipt of the written revocation.
6. Release of Liability
The Client hereby releases, discharges, and holds harmless Bridge Performance LLC and its representatives from any and all claims, demands, and liabilities related to or arising from the use of the media as stated in this Agreement.
7. Governing Law
This Agreement shall be governed by and construed in accordance with the laws of the State of New York.
