We are pleased to announce our first summer camp of the season! Coaches will be covering drills and exercises based on skill level and age. Available dates: Monday July 7th to Friday July 11th. Morning Session: 9:30am to 11:30am. Afternoon Session 1:30pm to 3:30pm. Location: 600 Courtney Park Drive. Please fill out the registration form, sign waiver, and complete payment to book your child's spot.

Informed Consent and Acknowledgement

At OLE FC, your child’s safety, growth, and enjoyment are our top priorities. By signing below, I give permission for my child to participate in all activities organized by OLE FC during the selected camp session.

I understand that sports—especially soccer—can involve physical activity and may carry some risk of injury. While OLE FC takes every precaution to ensure a safe and fun environment, I acknowledge that accidents can happen. These risks may include, but are not limited to, minor injuries like sprains or bruises, and in rare cases, more serious injuries.

By allowing my child to take part, I agree not to hold OLE FC, its coaches, staff, volunteers, or facility partners responsible for any injuries that may occur during travel to, from, or while participating in the camp.

I appreciate the positive impact OLE FC aims to have on every player, and I trust that my child will have a rewarding and enriching experience both on and off the field.
Medical Release and Authorization

Your child’s safety is our top priority, both on and off the field. In case of an emergency, we want to make sure they can get the care they need—even if you're not immediately reachable.

Please take a moment to read and sign this form so we know we have your permission to act quickly and responsibly if your child ever needs medical attention during a game, practice, or team activity.

Medical Permission

As the parent or legal guardian of the player named, I give permission for qualified medical professionals to provide care for my child in the event of an emergency when I can't be reached right away.

This includes allowing:
- First aid to be given by coaches, team parents, or other trusted adults
- Emergency treatment by medical staff if necessary
- X-rays, basic medical tests, or minor procedures if needed
- Efforts to reach me before anything major is done (unless it's urgent)
I understand that this permission is only for the current Ole FC season and is meant to help protect my child’s health and well-being when quick decisions are needed.
Media Release Waiver

As part of the Ole FC family, we love capturing the fun, energy, and teamwork that our players bring to the field. From time to time, we take photos and videos at practices, games, and team events to celebrate our players and share the joy of the game with our community.

From time to time we will be filming or photographing sessions for the following purpose:
- Our website and social media pages
- Team newsletters or announcements
- Flyers, posters, or club promotional materials
- Local news stories or highlight reels
These images are used to highlight the great things happening at Ole FC and to help grow a positive, supportive soccer community
