Pharmacy-led Medication History Survey
  • You are invited to take part in a survey from the TSHP Clinical Practice Section. Your participation is voluntary, and it is totally up to you to decide whether you want to participate. Even if you decide to be part of the study now, you may change your mind and stop at any time.

    The purpose of this study is to assess the landscape of pharmacy-led medication history programs in institutions across Texas and capture “good-catches” discovered because of medication histories.

    If you agree to fill out the short online survey, you will be asked basic information about your institution, if you have a medication history program, and experiences taking medication histories. It should take approximately 10 minutes to complete.

    There are only minor risks you might experience from being in this study. The primary risk that may occur is threat to anonymity. Institution name is only collected to ensure no duplicate surveys from the same institution. Of note, the survey includes opportunities for free/open-ended responses relating to “good catches” as a result of medication histories; please do not include any identifiable information in these responses. Survey answers will be stored in a password protected folder accessible only to the study team.
