WAIVER AND RELEASE OF ALL CLAIMS. PLEASE READ CAREFULLY AND BE AWARE THAT IN REGISTERING YOURSELF OR YOUR MINOR CHILD/WARD FOR PARTICIPATION IN THE LAKE COUNTY LIGHTNING OR ILLNIOIS LIGHTNING PROGRAMS, YOU WILL BE WAIVING AND RELEASING ANY AND ALL CLAIMS FOR INJURIES YOU OR YOUR CHILD/WARD MIGHT SUSTAIN FROM PARTICIPATING IN THE PROGRAM.

I/WE understand that there are certain risks of injury inherent in my/our child/ward's participation in the game of baseball, baseball practice and baseball tryouts. Having full knowledge of such risks, I/we voluntarily assume them on my/our own accord. I/we further agree to hold JERROLD FOX, LAKE COUNTY LIGHTNING, ILLNIOIS LIGHTNING, FREMONT TOWNSHIP, VILLAGE OF HAWTHORNE WOODS, AND ALL PARTICIPATING communities, facilities, organizations and their representatives harmless of any responsibility, financial or otherwise, due to any injuries received while playing or participating or traveling to and from events excepting in the case of gross negligence or an illegal act. In such case I/We will seek remedy only against the offending party.

In the event of any emergency, I authorize LAKE COUNTY LIGHTNING, ILLNIOIS LIGHTNING, FREMONT TOWNSHIP, VILLAGE OF HAWTHORNE WOODS, or other communities, facilities or organizations to secure from any licensed hospital, physician and/or medical personnel any treatment deemed necessary for me or my minor child/ward’s immediate care and agree that I will be responsible for payment of any and all medical services rendered.

I/WE hereby grant and authorize LAKE COUNTY LIGHTNING, ILLINOIS LIGHTNING, FREMONT TOWNSHIP, VILLAGE OF HAWTHORNE WOODS the right to take, edit, alter, copy, exhibit, publish, distribute and make use of any and all pictures or video taken of my/our child/ward to be used in and/or for legally promotional materials including, but not limited to, the LAKE COUNTY LIGHTNING, ILLNIOIS LIGHTNING, FREMONT TOWNSHIP, VILLAGE OF HAWTHORNE WOODS websites, newsletters, flyers, posters, brochures, advertisements, fundraising letters, press kits and submissions to journalists, social networking sites and other print and digital communications without payment or any other consideration. This authorization extends to all languages, media, formats and markets now known or hereafter devised. This authorization shall continue indefinitely, unless I/We revoke said authorization in writing.

I/We understand and agree the at these materials shall become property of the LAKE COUNTY LIGHTNING, ILLNIOIS LIGHTNING, FREMONT TOWNSHIP, VILLAGE OF HAWTHORNE WOODS and will not be returned.

I/We hereby hold harmless, and release JERROLD FOX, LAKE COUNTY LIGHTNING, ILLNIOIS LIGHTNING, FREMONT TOWNSHIP, VILLAGE OF HAWTHORNE WOODS from all liability, petitions, and causes of action which I/We, my/our heirs, representatives, executors, administrators, or any other persons may make while acting on my/our behalf or on behalf of my/our estate.
