I hereby agree to participate in this dance program given by JUDI T. DANCE STUDIO, INC. upon the understanding and condition that:
1. I recognize the risk of injury inherent in any exercise or dance program and am participating upon the express agreement and understanding that I am hereby waiving and releasing the company from and against any and all claims, costs, liabilities, expenses or judgments including but not limited to attorney's fees and court costs arising out of my participation in the company's programs, except for injury directly resulting from gross negligence or willful misconduct on the part of the company.
2. I hereby agree that the student and his/her parent/guardian grant permission to use his/her photographs, speeches, and/or videos for publicity purposes. Photos or videos may appear in studio publications, on the website or on the studio Facebook and Instagram page. You can be assured that all photos are done in a professional and appropriate manner. Thus, I hereby grant Judi T. Dance Studio permission to use my child's photograph and/or videotaped image for the purposes mentioned above.
3. Payment must be paid in order for spot to be reserved and can be made by cash, check, PayPal or Venmo to Judi T Dance Studio. We also accept Visa, Mastercard, Discover and American Express for all payments.
By signing, I hereby execute and deliver this waiver and release to permit my child to participate in it's program.
