I understand this is a contract for the named student and is for the above days/pick-up times. I understand payment is made through the school's parent portal (FACTS) or directly to the school's business office. I have read the information packet for the program and understand the required program rates, late pick-up fees, request for changes, and program details. By electronically selecting the box below, I agree that I am signing this contract and committing financially to the days selected above. The parents are responsible for payment of these days regardless of whether or not the child is in attendance at the program. I understand that days cannot be changed without a new registration form and signed contract. Requests for changes to this contract require a two week notice.
