By completing this volunteer application for Next Generation Playground (NGP), I certify that all information I have provided is true, complete, and accurate to the best of my knowledge. I understand that any false or omitted information may result in denial of my application or termination of volunteer status.

I acknowledge that volunteering with NGP is voluntary and does not create an employment relationship. Either party may end the volunteer relationship at any time, with or without cause or notice.

I understand that upon completion of this application and required training, I may be asked to complete a background check and additional screenings through Confirmify, which I will be required to authorize in order to volunteer with NGP.
