Photo Release: I give permission for Lyndee Lee/Sewing Jungle/Kids Can Sew and Learn To Sew Kids (sewing curriculum used in many camps/classes) to use pictures of my child (with regards to sewing) for advertising purposes, social media/digital/print/blogs etc. Names will NOT be used.

I understand I must give 30 days notice in writing should I choose to discontinue classes prior to May.
Tuition is based upon an AVERAGE of 3 classes per month throughout the school year (September-May) and broken down into 9 equal monthly payments. I have five to six make-up classes built into the schedule, so if I have to cancel classes for any reason, no additional make-up class will be offered.
Students are guaranteed 27 classes to be offered throughout the school year (Sept-May). This is an average of 3 classes per month. I have scheduled an extra 5 to 6 classes. If I need to cancel a class for any reason, no proration or make up classes will be offered unless it drops below the 27 guaranteed classes for the school year. Makeup classes are not offered for missed classes.
I understand the instructor/s and/or Lyndee Lee will do their best to prevent an accident from occurring, but I also understand that there is a risk of injury associated with participation in sewing and agree to waive any and all claims of liability and hold harmless Sewing Jungle and/or Lyndee Lee and/or any other instructors in the event that such an injury may occur to my child. Further, in the event of an accident or injury, when parent, legal guardian or emergency contact is not able to be reached, I give permission for Lyndee Lee or any other instructor to procure medial attention if she/they feel it necessary. Additionally, I understand my child may use any/all of the following items, and there is a risk of injury in doing so and agree to waive any and all claims of liability and hold harmless Sewing Jungle and/or Lyndee Lee and/or any other instructors in the event of injury. These items include, but are not limited to sewing machines, scissors, pins, needles, hot irons, and hot glue guns. Refunds are not offered for classes or camps.
