Policies & Waiver Forms - Chicago Pop Up

 Please read carefully and thoroughly.

 

POLICIES APPLY TO ALL SERVICES

I understand that this is a summary of policies and that these policies apply to all services, not only to the services that I will be receiving today.

 

ARRIVAL

+Arrive with clean/makeup-free eyes/brows for your lash or brow appointment. Allow additional time for traffic and parking. Please aim to arrive 5-15 minutes early to allow yourself time to park and walk up. Please add any additional time if you need to use the restroom, remove contacts, etc. If you arrive early feel free to wait in the waiting area.

 

TARDINESS POLICY

+Please arrive on time of your scheduled appointment. This will ensure you get the entire time of the service. Time is very precious, the more time we have together, the more time we have to work on you to achieve the best results.

+If you arrive 15+ minutes late; your appointment time will be cut down that amount, which will not give you the fullest result. If you show up 15+ minutes late, we can go through with the appointment. If you are late and choose to cancel or if we decide we don't have enough time to finish your appointment to do our best work with the time that is left, then we will cancel your appointment and a charge a 100% cancellation fee. We are booked back to back and need the entire appointment to do the best work. We have a very strict late policy. Your appointment will not be extended, if you are late. 

 

CANCELLATION & RESCHEDULING POLICY

+ Goodie Beauty understands things happen unexpectedly, but your appointment time is reserved especially for you. Appointments are scheduled back-to back for 1-2 days, please understand that by cancelling last minute a fee is enabled because we miss the opportunity to fill that slot with another client on our waiting list.

+ We respectfully ask you to give a 72 hour cancellation notice, so we can kindly give up your appointment slot to another client. A late notice given 1-24 hours prior to the scheduled service will be charged 50% of the service price to the card on file. A client who cancels/reschedules after their appointment start-time or doesn't show up to their appointment or will be charged 100% of the services booked.

+ If your appointment is booked under 24 hours and decide to cancel, you will be responsible for a 100% fee of the services booked.

Regardless of unforeseen circumstances that may arise, which could force you to cancel/reschedule your appointment, please understand that such changes affect not only your artist, but other clients as well. As we are respectful of your time, we ask that you please respect our time as well. 

DEPOSITS

+ If a deposit is required, you will be required to pay a non-refundable 50% deposit in advance. The deposit is added towards the balance of your appointment. The day of appointment you will need to pay the remaining balance. Deposits are to be paid for that specific appointments’ date and time. If you cancel or reschedule your appointment to another time that same day or to another calendar date, your deposit will be forfeited. Deposits are non-transferable. You cannot use a deposit of one cancelled/rescheduled appointment and add it towards a new appointment. If your deposit is not paid, we have the right to cancel your appointment.

 

SICKNESS OR FAMILY EMERGENCY

+ If you, or another person in your household has an infectious or contagious illness, please contact us as soon as possible to reschedule your appointment for a later date. For your safety and that of the staffs' household and other clients PLEASE DO NOT come to your appointments sick. If it is assumed you are currently sick, your appointment may be cut short or cancelled.

 

TALKING

+ We kindly ask that there should be no talking while applying lash extensions or during a lash lift service. Talking moves the pads which can cause rubbing in the eye that can lead to bruised and sore eyes. Talking can also cause eyes to open slightly which causes chemical burns. Feel free to bring some headphones or take a lash nap.

 

CHILDREN

There are sharp tools and chemicals throughout the salon. Some services require for your eyes to be closed for long periods of time. We care a lot about the safety of the little ones, we ask that that no children are to be brought to any appointments for their safety. If you have any questions, please feel free to contact us.

 

PERMISSION TO USE PICTURES

+ I consent to let Goodie Beauty take, publish and reproduce photos of me, my face, my eyes and/or eyelashes/eyebrows, both before and after the procedure, for any marketing/advertising, social media, education, or other purposes whatsoever, including the right to retouch these photographs as deemed necessary. I further expressly assign any copyright in these photographs. I also grant consent to use my images and likeness as contained in these photographs.

 

REFUND POLICY

+ You are paying for artist time, product and other expenses used to provide you with a service. All sales are final. No refunds or exchanges will be given for any reason on any and all physical & digital products, services, gift cards, deposits, memberships, subscriptions, packages, and appointment policy fees.

 

GIFT CERTIFICATES

+ Goodie Beauty Gift Certificates should be treated as cash. All sales are final; Gift Certificates are non-transferable and cannot be exchanged or refunded for any reason. Gift Certificates contain value that may be used to purchase services and products, however they cannot be used to purchase other gift certificates. Please present your Goodie Beauty Gift Certificates upon checkout and your sale will be deducted from your cards balance. All remaining balances must remain on your gift certificates; cannot be redeemed for cash. Service prices are subject to change.

+ To provide maximum protection to your credit card account, and to allow time for your order to process, our printable Gift Certificates become active and may be redeemed 24 hours after purchase.

+ Goodie Beauty is not responsible for certificates that are lost, stolen, damaged or used without authorization. Goodie Beauty certificates are the property of Goodie Beauty Please call 305-929-3719 for assistance with your Goodie Beauty Gift Certificate.

 

OTHER MATTERS OF IMPORTANCE

At Goodie Beauty the work we do is an art, not just a service. We have trained extensively with nationwide artists to provide you with a luxury service. We have worked on almost over 3000+ clients all throughout New York, New Jersey, California, Chicago and Miami with over 8+ years of experience. We are not the type of artists that focuses on quantity or getting people in and out, we focus heavily on quality services using the highest grade of products and the best skill sets in the industry by always staying consistent with what we provide. All of our clients appreciate that we put our all into every service whether its your first time or 5th time. The skills we have attained through many hours of training, practice and ongoing advanced courses are to be respected and by signing this form you are fully aware of our expectations as a client- just as we are aware of your expectations of us as artists.

CREDIT CARD HOLD

+ As part of our policies, it is required to hold a card on file to secure your appointment in our schedule. Without a card on file, your appointment will not be booked.  

By providing my card, I, authorize Goody/Goodie Beauty LLC to charge my credit card on file for agreed upon purchases/services. By providing a card on file I authorize Goody/Goodie Beauty LLC to charge my card on file for cancellation, rescheduling & no-show fees according to our policies above. I understand that my information will be saved to file for future transactions on my account.

CLIENT WAIVER (FOR ALL SERVICES)

+ This agreement will remain in effect for this service and/or procedure and all future appointments/follow-ups conducted by my technician or any other technician conducting business at Goodie Beauty LLC.

+ I herein signed, release, give up, acquit, and discharge Goodie Beauty LLC and or anyone affiliated there to including any partnership, corporations, or company associated with said individual from any claims, injuries or damages of any nature. I agree to pay any costs of legal services necessary to enforce said release. I further agree that this release shall be in contemplation of any possible damages, either known or unknown at the signing of this release and said damages are specifically waived following the signing of this release. I further agree that in the event any litigation ensues, it shall be placed before the American Arbitration Association or some other such arbitrator for resolution. I agree that in the event a decision is determined in favor of one party over the other, the prevailing party shall be entitled to reasonable attorney fees and costs as set by the arbitrator. 

+ I further agree to hold Goodie Beauty LLC professional nameless and harmless from any and all injuries or damages that may result from any/all service(s) performed, and without liability of any kind whatsoever for the responses to products used for the service(s) may vary.I release Goodie Beauty LLC from any responsibility for pre-existing conditions I have not revealed, or any consequential change to those conditions that arises after the service(s) performed. I understand I am responsible for any medical treatment I may need to receive because of getting any/all service(s).

+ I release Goodie Beauty LLC, its employees, contractors, representatives, and partners from any responsibility for any and all conditions that I have revealed or not reveal, or any loss or harm whatsoever. I agree that this constitutes full disclosure, and that it supersedes any previous verbal or written disclosures. I accept full responsibility for these and any other complications, which may arise or result during or after any and all services , which are to be performed at my request.

This agreement will remain in effect for any and all services or procedures and all future follow-ups. I read English and understand that this consent agreement is legal and binding. I have read and fully understand all information in this agreement. I certify that I have read, and fully understand, the above paragraphs and that I have had and or will have sufficient opportunity for discussion or to have any questions answered. I do not hold the professional(s) at Goodie Beauty LLC responsible for any of my conditions that were present, but not disclosed at the time of this procedure, which may be affected by the service(s) and procedure(s) performed today and all future appointments. I agree that I will not in any way hold the Goodie Beauty LLC, or any of its employees or representatives, responsible for any loss or harm that may result, including serious bodily injury or harm.
