I, the undersigned parent or legal guardian, give permission for my child to participate in the youth event listed above.
I understand that participation in youth activites may involve certain risks. I voluntarily assume these risks on behalf of my child and acknowledge that reasonable precautions will be taken to provide a safe environment.
I release and hold harmless the organizers, leaders, volunteers, staffers, and the nonprofit organization from liability for injuries, losses, or damages arising from participation in this event, except as otherwise provided by law.
In the event of a medical emergency, I authorize event leaders to obtain emergency medical treatment for my child if I cannot be reached. I understand that I am responsible for any medical expenses incurred.
