By signing below, I confirm that I am enrolling in the AZ Cert Prep CDA (Child Development Associate) Credential Prep Course using Empowerment Scholarship Account (ESA) funds. I understand that I will be issued an invoice and must submit it for payment throuugh ClassWallet before my enrollment is confirmed. I agree to follow all course expectations, and I acknowledge that my training hours will not begin until payment is received. 

By signing below, I confim that I am enrolling in the AZ Cert Prep CDA (Child Development Associate) Credential Prep Course through the WIOA program. I understand that I must complete and submit all required WIOA paperwork before I can begin the course. I agree to comply with all documentation requests and verify attendance as needed for WIOA compliance. 

By signing below, I confirm that I am enrolling in the AZ Cert Prep CDA (Child Development Associate) Credential Prep Course as a self-paying student. I understand that I must complete payment through Stripe before gaining full access to the course content. I agree to follow the course expectations and complete the 120 hours of training to receive a certificate of completion.
