By signing this form, I confirm the following:
- My employment is directly related to my major and exceeds 20 hours per week.
- I will report any changes to my address, phone number, employer, employment status, name, visa status, school, or degree level within 10 days.
- I understand that I am responsible for maintaining my F-1 status, including unemployment limits (90 days for OPT; additional 60 days for STEM OPT).
- If on STEM OPT, I will complete and submit I-983 self-evaluations at 12 and 24 months.
- I will confirm or update my address and employment every 6 months during STEM OPT.
