Terms and Conditions
By submitting this application to The House of Wellness, I understand and agree to the following terms and expectations should I be selected to join the team:
Independent Contractor Status
I acknowledge that I may be offered work as an independent contractor, not as an employee. This means I am responsible for my own taxes, liability insurance (if applicable), and compliance with any state licensing requirements.
Booth Rental vs. Commission Structure
I understand that The House of Wellness offers booth rental and/or commission-based opportunities. The final agreement (including rental rates or percentage split) will be discussed and mutually agreed upon before beginning any services.
Licensing & Legal Compliance
I certify that all professional licenses and certifications I’ve listed are current and valid in the state of Georgia. I understand that I am responsible for maintaining my license and following all applicable laws, including sanitation and health regulations.
Professional Conduct & Standards
I agree to maintain a professional demeanor at all times, including respecting client confidentiality, keeping the treatment area clean and organized, and treating all staff, clients, and guests with respect.
Scheduling & Client Care
I agree to arrive on time for scheduled appointments, provide high-quality service, and follow proper client intake and documentation procedures. I also agree to notify management promptly of any scheduling issues or conflicts.
Social Media & Branding
I understand that if I use social media to promote my services, I may be expected to tag or mention The House of Wellness in posts, and any branding or promotional materials must align with the spa's professional image.
Termination of Agreement
The House of Wellness reserves the right to terminate any rental or commission agreement for failure to uphold the policies outlined above, or for conduct deemed unprofessional, unethical, or harmful to the reputation of the business.
