Our Organization, BEL-C is dedicated to providing holistic, safe, and engaging learning support for children, youth, and the caregivers who guide them — at home and in community.
At BEACONS Early Learning Collective (BEL-C), we support learning beyond the classroom through creativity, mentorship, and hands-on exploration.All BEL-C programs follow a shared learning cycle — Play • Think • Create • Reflect — adapted thoughtfully for each child’s age, stage, and learning goals.
This intake form helps us understand your child’s needs and identify the program pathways that may be the best fit.

The information collected here is used across BEL-C programs serving children from toddlerhood through adolescence, with age-appropriate expectations and supports.

You will complete this intake once per child. After submission, you’ll be guided to a program-specific registration step that includes details on schedules, pricing, participation expectations, and policies.

Please note: Submitting this intake does not confirm enrolment. Registration is confirmed only once program-specific requirements are met.

Enrolling Siblings

We love welcoming siblings to BEL-C and value the relationships they bring into our learning community.

Families enrolling more than one child in the same schedule are eligible for a 10% sibling discount.

To apply the sibling discount, please complete this form for each child. After registering your first child, contact the BEL-C Enrollment Team to receive a discount code for your additional child.

BEL-C programs operate as enrichment and learning experiences and are not licensed childcare services unless explicitly stated.

Submission of this form authorizes BEL-C to use the information provided for program registration, safety planning, and communication.
🛡️ Health, Safety & Supervision

BEACONS Early Learning Collective (BEL-C) follows all applicable municipal, provincial, and facility health and safety guidelines, including those set by the City of Ottawa and public health authorities.

Our team is committed to providing a safe, caring, and inclusive environment for every child. Daily safety routines and visual checks are used to maintain clean, secure, and child-friendly spaces during all programs.

We are committed to:

Monitoring children closely during all activities (indoors and outdoors)
Maintaining secure spaces, checking equipment, and addressing hazards
Following health, hygiene, and allergy-aware practices
Using clear communication procedures in the event of illness, injury, or emergency

Parents/guardians will be contacted promptly if any health or safety concerns arise.
👕 Dressing for the Day

Children should arrive dressed in:
- Comfortable, art-friendly clothing that can get messy
- Non-slip socks or clean indoor shoes for indoor use
- Please also dress your child appropriately for the weather, as outdoor time may be included when conditions allow.
🎒 Personal Belongings

Please label all personal items, including:
- Water bottles
- Clothing
- Bags and containers
While our team supports children in keeping track of their belongings, BEL-C is not responsible for lost or misplaced items.
🤒 Health & Wellness

If your child is feeling unwell, we kindly ask that they stay home to help protect the well-being of the group.

Children must be well enough to participate fully in the day.

If a child becomes ill or is unable to participate safely during a session, parents/guardians will be contacted to arrange early pick-up.
📸 Photo & Media Consent

We may occasionally document children’s artwork or activities to share program updates with families.

Photos are used respectfully. If you choose to opt out, faces will be blurred or images will not be used.

You may update or withdraw photo consent at any time by notifying us in writing.
