  • This waiver shall be valid from May 1, 2025 through May 31, 2026, and shall apply to all individuals participating in any volunteer activities organized or sponsored by DB CAPA (Diamond Bar High School Chinese American Parents Association), including but not limited to both in-person and online activities.

  • Participation in such volunteer programs is contingent upon the parent(s) or guardian(s) of the participating DBHS student being active members of DB CAPA during the 2025–2026 academic school year.

  • Print the name of all family members who may participate in any Chinese American Parent Association - Diamond Bar (CAPA) sponsored events (including parent/guardian and minor child(ren)): 

  • The undersigned parent(s) or guardian(s) assume all risks in connection with the participation of all individuals listed above in any and all of the CAPA sponsored activities. 

    I attest and verify that all individuals listed above are physically fit and able to participate in any CAPA sponsored activities. Further I acknowledge that is it my responsibility to understand any inherent risks associated with CAPA sponsored activities and communicate those risks to all individuals named above. 

    I do hereby certify that to the best of my knowledge and belief all individuals named above are in good health. In the event that I, or other parent/guardian, cannot be reached in an emergency, I hereby give permission to secure proper treatment for my child(ren). I/we do hereby consent to whatever x-ray, examination, anesthetic, medical, surgical or dental diagnosis or treatment and hospital care are considered necessary in the best judgment of the attending physician, surgeon or dentist and performed by or under the supervision of the medical staff of the hospital or facility furnishing medical or dental services. It is further understood that the undersigned will assume full responsibility for any such action, including payment of costs. 

  • I/we, as parent(s) or guardian(s) of the minor(s), do hereby, for my child/children, myself, my heirs, executors and administrators, release and forever discharge and hold harmless the CAPA and all officers, directors, employees, agents and volunteers of the organizations, acting officially or otherwise, from any and all claims, demands, actions or causes of action which in any way arise from the participation of any individuals listed above in any CAPA sponsored activities. 

    By signing below, I confirm that I have carefully read and fully understand its contents. I am aware that this is a release of liability and signed it of my own free will. 

  • ADULT PARTICIPANT / VOLUNTEER WAIVER, RELEASE, AND INDEMNITY AGREEMENT for 05/01/2025-5/31/2026

  • PARENT/GUARDIAN WAIVER: I voluntarily agree to participate in CAPA sponsored events and activities held during the school year. I recognize that the CAPA has not undertaken any duty or responsibility for my safety and I agree to assume the full responsibility for all risk of bodily injury, death, disability, and property damage as a result of participating in the CAPA sponsored events and activities. I recognize that these risks will vary based on the event and activity, and understand it is my responsibility to be aware of the risks before participating. I attest and verify that I am mentally and physically fit and able to participate in CAPA sponsored events and activities. By my signature below, I hereby state that I understand there are risks involved in participating in CAPA sponsored events and activities and willingly and voluntarily accept these risks. By my signature, I hereby surrender any right to seek reimbursement from the CAPA and all of their officers, directors, members and volunteers for injury sustained and liability incurred during my participation in CAPA sponsored events and activities. 

    PARENT/GUARDIAN WAIVER 

    This section sets forth the responsibilities and understandings of the volunteer and of the CAPA regarding parent/guardian’s participation in volunteer programs partially or wholly coordinated by the CAPA during the school year. 

    The parent/guardian and the CAPA agree as follows: 

    1. The parent/guardian performs the service of the parent/guardian’s own free will, without promise, expectation, or receipt of remuneration. The parent/guardian is not an employee or agent of the CAPA for any purpose and the volunteer’s services are not controlled nor mandated by CAPA. 

    2. The parent/guardian understands and agrees that it is possible that the volunteer may be injured or otherwise harmed during volunteer service due to accidents, acts of nature, the parent/guardian’s negligent or intentional acts, or the negligent or intentional acts of others; that while the CAPA has taken some steps to reduce the chances of injuries or harm to the volunteer, that the CAPA has no control over most risks, and, thus, cannot and does not guarantee nor take any responsibility for the safety of the parent/guardian or the parent/guardian’s property while the parent/guardian is engaged in volunteer service; and that the parent/guardian must take full responsibility for himself or herself and assume the risk of harm or damage while serving by taking all necessary and reasonable precautions and acting in a manner that will help protect himself or herself and his or her property. 

    3. The parent/guardian agrees to waive and release the CAPA, including all of their officers, directors, members, and volunteers from any and all potential claims for injury, illness, damage, or death which the parent/guardian may have against the CAPA that might arise out of the volunteer’s service and to hold the CAPA harmless there from. 

    4. The parent/guardian agrees and understands that injuries or losses to others, such as co-workers or the person(s) being helped, may occur as a result of the parent/guardian’s negligent or intentional acts during volunteer service, and that to avoid such harm, the volunteer must exercise care and act responsibly in serving others. 

    5. If any injury or loss to another does occur due to the volunteer’s intentional actions or due to volunteer’s negligent actions arising outside of the scope of the volunteer’s activities, the volunteer must accept the liability for and repair, or make reparations for, the harm done. 

    6. In projects where the volunteer will be transporting others in a non-CAPA owned vehicle, the volunteer will be required to provide proof of automobile insurance in order to participate. 

    7. Since volunteers are not the CAPA employees, the CAPA does not provide workers’ compensation coverage for injuries or illnesses to the volunteer arising out of volunteer activities. 


    I understand that the materials and tools provided by the CAPA are and remain the property of the CAPA, and I agree to return these tools and any remaining materials to the CAPA at the end of my volunteer service. 

    By signing below, I confirm that I have carefully read this document and fully understand its contents. I am aware that this is a release of liability and signed it of my own free will. 

  • Image Release Authorization

  • By signing this release form, I authorize Chinese American Parent Association (CAPA)  to use the following personal information:

     
    (1)  My picture – including photographic, motion picture, and electronic (video) images.
    (2)  My voice – including sound and video recordings.
     
    I hereby grant to CAPA, its subsidiaries, licensees, successors and assigns, the right to use, publish, and reproduce, for all purposes, my name, pictures of me in film or electronic (video) form, sound and video recordings of my voice, and printed and electronic copy of the information described in sections (1) and (2) above in any and all media including, without limitation, cable and broadcast television and the Internet, and for exhibition, distribution, promotion, press conferences, meetings, hearings, educational conferences and in brochures and other print media. This permission extends to all languages, media, formats and markets now known or hereafter devised. This permission shall continue forever unless I revoke the permission in writing.
     
    I further grant CAPA all right, title, and interest that I may have in all finished pictures, negatives, reproductions, and copies of the original print, and further grant CAPA the right to give, transfer, and exhibit the print in copies or facsimiles thereof, for marketing communications, or advertising purposes, as it deems fit.
     
    I hereby waive the right to receive any payment by signing this release and waive the right to receive any payment for CAPA the use of any of the material described above for any of the purposes authorized by this release. I also waive any right to inspect or approve finished photographs, audio, video, multimedia, or advertising recordings and copy or printed matter or computer generated scanned image and other electronic media that may be used in conjunction therewith or to approve the eventual use that it might be applied.

    If I am bringing a child under 18, I am signing it also for my child.

    I have read this document, and I am signing it freely. No other representations concerning the legal effect of this document have been made to me.

  • ACCEPTANCE OF TERMS FOR VOLUNTEER PROGRAM

    By participating in the volunteer program (hereinafter referred to as "the Program"), you (hereinafter referred to as "the Volunteer") expressly acknowledge and agree to abide by the terms and conditions set forth in this agreement. The Volunteer acknowledges that they have carefully read, understood, and accepted all the terms, policies, and guidelines outlined above, as well as any additional terms and conditions referenced or linked within. This acceptance includes but is not limited to the program requirements, code of conduct, privacy policy, and any other relevant policies or agreements made available to the Volunteer. Furthermore, the Volunteer acknowledges that their continued participation in the Program constitutes an ongoing acceptance of these terms and any future modifications or updates thereof. Should the Volunteer not agree with any provision of this agreement, their sole recourse is to immediately cease their participation in the Program. By accepting these terms, the Volunteer represents that they have the legal capacity and authority to enter into this agreement.
