WAIVER AND RELEASE- PLEASE READ CAREFULLY

I realize that my children, and/or I are participating in a martial arts program and training. There is a possibility of injury to others, or me or my children being injured by others including instructor(s) and/or other students, who are participating in the same or related activity. I, for myself, my heirs, administrators, representatives and assigns and minor children, waive and forever release any and all rights and/or claims for any and/or all injuries or damages against the American Combat Aikido, LLC a/k/a/ Aikido Arts Institute, its instructors, and/or against any other person or entity involved in the activity and will not seek to hold any of these persons or entities liable for any injury or damage suffered by me or my minor children. I understand that I myself am protected by this provision against being sued by another student who signs this form if an injury results from my participation just the same as if I am injured by someone else. I accept full responsibility for any personal injury to myself or my minor children and for any damage to personal property. I sign below for myself, and any minor children, enrolled or participating in the activities at the dojo or anywhere else instruction is held by American Combat Aikido, LLC a/k/a/ Aikido Arts Institute or its instructors. I understand that neither I nor my minor children need to become a member of this dojo, train at this facility, receive martial arts instruction at this facility or anywhere else, or participate in martial arts training or activities conducted by American Combat Aikido, LLC a/k/a Aikido Arts Institute or its instructors; therefore, I am perfectly free to not sign this form and not join this dojo or continue as a member or participate in training. I am making the choice to sign this form freely and voluntarily. There is information on this novel corona virus, COVID 19, the risks associated with exposure, and recommendations on how to minimize the risk of contracting the virus available from sources such as the CDC website https://www.cdc.gov/coronavirus/2019-ncov/index.html and the New Jersey website, https://covid19.nj.gov/. Despite any and all precautions, transmission of the novel corona virus (COVID 19) remains possible through exposure to airborne particles, surface contact, or person-to-person contact. Martial arts training and your participation in classes will take place indoors. Person-to-person contact, airborne exposure, and surface contact will occur. By signing below, I acknowledge that I understand and knowingly and voluntarily accept the risks associated with exposure to conditions that could result in me becoming infected with the novel corona virus (COVID 19) by and through my participation in the activities that take place in the facility or dojo. By signing below, I make such acknowledgement for myself and any minor child.

I understand that neither I nor my minor children need to become a member of this dojo or train at this facility or receive martial arts instruction at this facility or any facility for that matter or participate in martial arts training conducted by American Combat Aikido, LLC a/k/a Aikido Arts Institute or its instructors; therefore, I am perfectly free to not sign this form and not join this dojo or participate in martial arts activities and training. I am making the choice to join

this facility or remain a member at this facility or participate in martial arts activities or training and sign this form freely and voluntarily, including the release that follows: I, for myself, my heirs, administrators, representatives and assigns, and minor children, waive and forever release any and all rights and/or claims for any and/or all damages against the American Combat Aikido, LLC a/k/a Aikido Arts Institute, its instructors, and/or against any other person or entity involved in the activity and will not seek to hold any of these persons or entities liable for any injury, illness or damage suffered by me or my minor children as a result of contracting the novel corona virus or COVID 19. I accept full responsibility for any injury, illness or damage to myself or my minor children as a result of contracting the novel corona virus or COVID 19. I sign below for myself, and any minor children, enrolled or participating in the activities at the facility or dojo or conducted by American Combat Aikido, LLC a/k/a Aikido Arts Institute or its instructors. I further realize I, or my child or children, are receiving instruction in a martial art that can cause injury or death to another human being. I or my child or children desire instruction in this art for self-improvement and also as a self-defense to be used only to protect my family and myself. I agree to hold harmless the American Combat Aikido, LLC a/k/a Aikido Arts Institute and its instructors from all liability that may result the use of martial arts techniques by me or my minor children.

ASSUMPTION OF PREXISTING INJURY STATEMENT

I the undersigned understand that there may be a possibility of re-injuring the injury stated on the front of this form. And that I may be injured in this training. I therefore accept and assume all risks involved (anticipated or unanticipated) resulting from my participation in this Aikido training. I further agree that the American Combat Aikido, LLC, a/k/a Aikido Arts Institute, it's officers, instructors, agents or other named parties, will be held harmless of any liability resulting from this or any future class or instruction on or off the premises.
