I/We, in consideration of my/our child's participation in Tranquiity Full Gospel Youth Camp, do for myself/ourselves assume all risks of personal injury or property damage sustained by my/our child arising out of the child's participation in TFGT Youth Camp activities such as sports, recreational activities including but not limited to the inflatables and release, aquit and forever discharge TFGT Youth Camp, TFGT Chuch, Pastors and leaders from any and all claims, damages, actions, or causes of action, for personal injury or property damage sustained by me/us or my/our child, in any way arising out of or in connected with my/our child's participation TFGT Youth Camp activities conducted on and off of the TFGT Church property. I/We hold TFGT Youth Camp/Church harmless from any and all liability resulting from any claim, demand, action or cause of action made by me/us or my/our child in connection with my/our child's participation in TFGT Youth Camp activities.

By my selecting "I Agree" below, I acknowledge that i have read, completely understand, and hereby agree to obey the rules and regulations of participation in all TFGT Youth Camp 2025 activities. I also agree to follow the leadership of the TFGT Directions, Counselors and Volunteers.
