Informed Consent and Acknowledgement
By submitting this request for assistance, I acknowledge and agree to the following:
1. Eligibility & Assistance: I understand that completing this form does not guarantee the approval of assistance. All requests are subject to availability, eligibility criteria, and the resources of Whiskers of Hope Pet Sanctuary. Assistance is provided based on the organization's capacity and priorities.
2. Information Accuracy: I confirm that the information provided in this form is accurate and truthful to the best of my knowledge. I understand that providing false or misleading information may result in my request being denied or assistance being rescinded.
3. Confidentiality: Whiskers of Hope Pet Sanctuary will keep all personal information provided in this form confidential, and it will only be used for the purpose of processing my request for assistance. I understand that my information may be shared with authorized parties as needed to facilitate assistance.
4. Liability: I release Whiskers of Hope Pet Sanctuary, its directors, its employees, volunteers, and partners from any liability that may arise from the provision of assistance or any subsequent use of the services or resources provided.
5. Use of Resources: I acknowledge that any pet food, supplies, or assistance provided by Whiskers of Hope Pet Sanctuary is for the sole use of the animals in my household. I agree not to sell, redistribute, or otherwise transfer any of the items received through this program to others.
6. Non-Sale Agreement: I understand that the pet food and supplies provided are solely for the care and well-being of my pets, and I am prohibited from reselling or distributing these resources for any commercial purposes.
7. Agreement to Terms: I understand that by submitting this request, I consent to the terms outlined above and agree to comply with any further instructions or requirements necessary for receiving assistance.
