  • EMERGENCY CONTACT INFORMATION: I authorize the school district to provide or secure any necessary emergency care for my child. It is the parent's legal responsibility to assume medical and transportation expenses for your child. Note that persons listed as emergency contacts are not authorized to pick up your child, unless listed in this section. Any person verified as a parent above and in the District's Student Information System is presumed to be authorized to pick up the student unless otherwise indicated.

  • Parents/guardians have the right to review the professional qualifications of their child's classroom teacher(s) including the licensing status, degree major, graduate degree(s) and the field of certification. This "right to know", available from your child's school, includes whether your child is receiving services provided by paraprofessionals and, if so, their qualifications.

    The Emergency Student Data Form governs early release withdraw of the student. The registering parent/guardian must sign/verify this form and is responsible for providing truthful and accurate information. If the student's parents are divorced or separated, the enrolling parent is responsible for providing information that is consistent with the most recent court order governing such matters as divorce, separation or custody. Additionally, please be advised that early release does not constitute an excused absence from class, students must bring a note in for their absences from any class even if they were picked up early. 

    Whoever knowingly makes a false statement in writing with the intent to mislead a public servant in the performance of his/her official duty shall be guilty of a misdemeanor of the second degree under Fla. Stat § 837.06, or whoever makes a false verified declaration is guilty of the crime of perjury, a felony of the third degree, under Fla. Stat. § 95.525, which are punishable as provided in Fla. Stat., §§ 775.082, 775.083 and 775.084

  • Behavior / Attendance Contract

  • BEHAVIOR

    To maintain student safety and a safe learning environment, all students at Palmetto Middle School are required to follow the following student behavior contract. Participation in school sponsored activities is contingent upon students demonstrating appropriate behavior and attitude. All students must abide by the rules and regulations set forth by the Miami-Dade County Public Schools Code of Student Conduct and the schoolwide guidelines and procedures which include:

    • A signed contract with parent and student signature must be on file.
    • Student must adhere to Palmetto Middle School’s uniform policy and dress code.
    • Student must not have a 3 in effort on a report card during the school year. 
    • Student must not have a D or F in conduct on a report card during the school year. 
    • Student must always wear PMS ID on their person while on the school campus.
    • Student must not have any financial obligations to the school, such as overdue books, lost textbooks, etc.
    • Student must not have a history (HISTORY = 3 or more) of written warnings or detentions given by any teacher.
    • Student must display proper cafeteria behavior consistently (i.e., remaining in seat, correct table manners, etc.).
    • Students who earned 6 or more indoor suspensions (CSI) and/or any outdoor suspensions will not partake in the 2nd nine-week activities, such as the dance, Turkey Bowl game tryouts, Turkey Bowl Day, and the Panther for a Day field trip to Palmetto High School, among other events.
    • In line with our commitment to a spirit of second chances, students who are interested in attending future activities (school and local field trips) must have zero outdoor suspensions and/or may not exceed 5 days of indoor suspension (CSI).
    • Additionally, for out-of-county or state field trips, students with outdoor suspensions at any point in the year will not be eligible to participate. These measures aim to encourage positive behavior and ensure a conducive and safe environment for all our students.

     

    ATTENDANCE

    The following attendance guidelines are in place based on Miami-Dade County Public School's Board Policy:

    1. Students in grades 6-12 who have 10 or more unexcused absences in an annual course or five or more unexcused absences in a semester course will be subject to the withholding of passing final grade(s).
    2. A student who has 20 or more cumulative tardies will not be allowed to participate in interscholastic competitions or performances for the remainder of the school year.
    3. The early release of students causes disruption to the academic performance of all students and may create safety and security concerns. No students shall be released within the final 30 minutes of the school day.

    Students that fail to comply with the Code of Student Conduct & have a Level 2 infraction and/or have referrals leading to Outdoor Suspension may result in being prohibited from participating in extracurricular activities & school functions.

    Acknowledgement and Receipt
  • Each parent/guardian of a student and each student enrolled in Miami-Dade County Public Schools must sign and return this page to the student's school to acknowledge that he/she has accessed the online version or obtained a copy of the Code of Student Conduct. In addition, this page serves as acknowledgement that you have reviewed the Code of Student Conduct with your child. Each school will maintain records of such signed statements.

    The online version of the Code of Student Conduct in English, Spanish, and Haitian-Creole can be located in the Parent Portal or by accessing through the following website address:

    If you do not have internet access to obtain a copy of the Code of Student Conduct, please visit your child's school to obtain a copy.
