PLEASE READ CAREFULLY. THIS IS A LEGAL DOCUMENT.
By signing and submitting this application, I understand and agree to the following on behalf of myself or my child (the “Participant”) in connection with participation in any Cooper International Academy programs, activities, housing, travel, or events (collectively, the “Program”):
1. Assumption of Risk & Release of Liability
I acknowledge that participation in the Program may involve physical, academic, residential, athletic, travel-related, and recreational activities that carry inherent risks of injury, illness, loss, or property damage. These include, but are not limited to, sports-related injuries, exposure to illness, accidents during off-site events or transportation, or issues related to boarding/residential life.
I voluntarily assume all such risks and release and hold harmless Cooper International Academy, its directors, staff, volunteers, agents, host families, affiliates, and any partnering organizations from any and all liability, claims, demands, or causes of action for injury, illness, damage, loss, or expenses, whether known or unknown, arising from or in any way related to participation in the Program.
2. Medical Consent
I hereby authorize Cooper International Academy staff, representatives, or medical personnel to seek emergency medical care, treatment, or hospitalization for the Participant in the event of injury or illness when I cannot be reached. I agree to be financially responsible for all medical expenses not covered by insurance. I confirm that the Participant will have valid travel medical insurance for the entire duration of the Program and will provide documentation as required.
3. Responsibility for Personal Property
I understand that Cooper International Academy is not responsible for the loss, theft, or damage of personal items brought to school, housing, travel activities, or off-site locations.
4. Media Release
I grant Cooper International Academy permission to photograph, record, or film the Participant during Program activities and to use such media for marketing, social media, promotional, or educational purposes without compensation. This release includes use in brochures, websites, videos, digital content, or public presentations. I understand that I may revoke this consent in writing if desired.
5. Governing Law & Jurisdiction
I agree that this agreement shall be governed by the laws of the State of California. Any legal dispute or claim arising from participation in the Program shall be resolved exclusively in the courts of San Bernardino County, California.
6. No Refunds Policy
By registering for the Cooper International Academy Spring/Summer Program, the Client acknowledges and agrees that all payments made are non-refundable. This includes, but is not limited to, deposits, training fees, housing fees, and any additional charges related to participation in the program.
Refunds will not be issued under any circumstances, including but not limited to:
- Voluntary withdrawal from the program
- Dismissal due to rule violations or conduct issues
- Injury, illness, or personal schedule conflicts
- Travel interruptions or changes in availability
Full financial commitment remains due regardless of attendance or early departure. By submitting payment or confirming participation, the Client agrees to these terms in full.
7. Acknowledgment & Certification
I affirm that all information provided in this application is true and complete. I have read, understood, and voluntarily agree to the terms of this Release, Waiver, Medical Consent, Media Release, and No Refunds Policy. If the Participant is a minor under age 18, I confirm that I am the parent or legal guardian and have the authority to provide this consent on their behalf.
