The Ulti-Mutt Academy
The Ulti-Mutt Academy values your pet and wants to ensure you that we will make every attempt to make your pet's grooming experience as safe and enjoyable as possible. During the grooming procedure, everyone's safety comes first, both the people and the animals.

You will only need to sign this form once, and it will apply to all pets you already own as well as those you may acquire in the future. Please do not hesitate to contact us if you have any questions!

Vaccines
All pets MUST be up to date on all vaccinations, and show proof of Rabies, Distemper, Bordetella & Town License. This is required in order to get grooming services. If your dog(s) attend camp and we already have their current records on file, you do not have to upload them again.

I certify that my pet(s) is/are current on all required vaccinations and is/are free from contagious diseases and parasites, including fleas and ticks. I have disclosed any known medical conditions, allergies, skin sensitivities, and behavioral concerns (such as anxiety, fear-based aggression, or a history of biting). I understand that failure to disclose this information may increase the risk of injury to my pet, other pets, or staff, and may result in additional fees or refusal of service.
Grooming can sometimes reveal a concealed medical condition or aggravate an existing one. This might happen while grooming or afterward. Please notify us of any current or previous problems so that we can keep an eye out for them. When you check the box here, the pet's owner will bear all medical fees for veterinarian care.
Matting & Shave-Down Policy
I understand that if my pet is heavily matted, The Ulti-Mutt Academy may need to shave the coat for humane and safety reasons. I acknowledge that removing mats can cause skin irritation, nicks, or redness, and may expose underlying conditions. I will not hold The Ulti-Mutt Academy or its staff responsible for any injury, irritation, or aesthetic outcome resulting from necessary demating or shave-down procedures.
Accidents
Although accidents are extremely rare when working with animals, they do occur. Grooming equipment is sharp, and while we employ extra caution and care in all cases, cuts, nicks, scratches, quickening of nails, and other issues are conceivable. When an animal is wiggling or moving around, this is most likely to happen. The safety and comfort of your pet is our first priority. In the unlikely event that an accident occurs, you will be alerted. I release The Ulti-Mutt Academy, its staff, and representatives from any liability, claims, or damages resulting from grooming
Authorization from a Veterinarian in Case of a Medical Emergency
In the event of a medical emergency, I authorize The Ulti-Mutt Academy to seek veterinary care for my pet. I understand that every attempt will be made to contact me first, then my emergency contact, but if we are unreachable, I authorize treatment to be provided at the discretion of the attending veterinarian.
Aggressive or Uncooperative Behavior
If my pet becomes aggressive, overly stressed, or poses a danger to staff or other animals, The Ulti-Mutt Academy reserves the right to stop the grooming process. I may still be responsible for a partial or full grooming fee depending on the time and work already completed. Muzzles may be used if necessary for safety, and I understand this is done humanely and professionally.
Cancellations/No Shows
We phone every appointment the day before to remind you of your appointment (please confirm that we have the correct number on file). If you are going to be late for your appointment for any reason, please let us know as soon as possible. All cancellations or reschedules require at least a 24-hour notice to provide us time to fill the spot with a customer from our waiting list. Grooming appointments will be held for 15 minutes after the stated drop-off time. Appointments that are more than 15 minutes late may be canceled due to high demand. A $25 fee will be charged for no-call-no-shows and appointments cancelled with less than 24 hours notice.
Photo and Media Release
I give permission for my pet(s) to be photographed or video recorded during their visit and understand that these images may be used for marketing, social media, or promotional materials by The Ulti-Mutt Academy.
Hold Harmless Agreement
I agree to release, hold harmless, and indemnify The Ulti-Mutt Academy, its owners, employees, and agents from any and all claims, damages, injuries, or losses arising from or related to services provided unless due to gross negligence. This agreement shall apply each time services are rendered.
I have reviewed this Service Contract for accuracy and understand the contents of this contract. I affirm that I am the rightful legal owner of the dog(s) for which services are being rendered. I authorize this contract to be valid approval for future grooming services, permitting us to accept telephone reservations for service without additional signed contracts or written authorization. I understand that pricing is subject to change. This agreement is valid for all pets that I currently own or will own in the future. I have read, and agreed to the above.
