CKYO Bravo String Orchestra Registration

By signing below, I (PARENT/GUARDIAN)* give permission for CKYO to add my student’s cell phone number to the Remind app for official communication. All text communication between CKYO and students will take place exclusively through the Remind platform, in compliance with state guidelines.

Section I: Medical Release and Authorization

I, as the parent or legal guardian of the minor listed above, give my authorization (via electronic signature) to the staff members of Central Kentucky Youth Orchestras, to administer first aid to the minor listed on this form for minor injuries such as scrapes and bruises. In case of more serious injury or illness, attempts will be made to contact me. CKYO is authorized, if determined necessary by CKYO, to seek emergency medical treatment for the minor listed here including transportation by ambulance to an emergency medical facility. I agree to be 100% financially responsible for the cost of said transportation and treatment. I certify that my child has sufficient accident/medical insurance coverage for any reasonably probable contingency.

This release is authorized and executed of my own free will, with the sole purpose of authorizing medical treatment under emergency circumstances, for the protection of life and limb of the named minor child, in my absence.

BY ACKNOWLEDGING AND SIGNING BELOW, I AM DELIVERING AN ELECTRONIC SIGNATURE THAT WILL HAVE THE SAME EFFECT AS AN ORIGINAL MANUAL PAPER SIGNATURE. THE ELECTRONIC SIGNATURE WILL BE EQUALLY AS BINDING AS AN ORIGINAL MANUAL PAPER SIGNATURE.

Section II: Release of Liability

I, the undersigned parent or legal guardian, authorize the named student ("Student") on this form to participate in the Central Kentucky Youth Orchestras program (“CKYO”). I acknowledge there are risks in any youth program.

In consideration of the right of Student to participate in the activities offered by CKYO, I agree to release, hold harmless and indemnify CKYO, its directors, officers, employees, volunteers, and agents from any and all liability, claims and causes of action arising out of or in any way connected with Student’s participation. If I am signing on behalf of a minor, I further agree to hold harmless and indemnify CKYO, its directors, officers, employees, volunteers, and agents from all liability, claims and causes of action that the minor may have arising from the minor’s participation in the activities of CKYO.

I understand that although the Released Parties may be present at performances and other CKYO activities (the “Activities”), the Released Parties will not be responsible for supervision of the Student outside of the rehearsal time. I am solely responsible for supervising the Student in connection with the Activities until Student checks into or is dismissed from the Activities. I and the Student shall comply with all applicable laws in connection with the Activities, including all laws concerning alcohol and minors.

I understand that the Student is responsible for his or her instrument and other personal property brought to CKYO activities. I agree that if I need insurance to cover the instrument, I will arrange for such insurance with my own insurance company. I hereby agree to indemnify, hold harmless and release from liability CKYO and Released Parties for damage or loss to such instruments or personal property.

I agree that both I and Student will abide by all CKYO and Venue safety rules. I also agree to contact the CKYO Executive Director or other CKYO staff immediately concerning any injury to student, damage to student’s property, violation of CKYO policies, Venue’s rules, or violations of law.

Section III: Media Release

During each season, musicians may be highlighted in efforts to promote the CKYO. For example, musicians may be featured in grant applications, newspapers, radio, TV, internet, DVDs, marketing displays, brochures and other types of media.

I, as parent or guardian of the minor listed below, hereby authorize CKYO representatives and CKYO authorized media organizations to photograph and record the student for use in audio, video, film, or any other electronic, digital, printed, or other media. This is with the understanding that neither CKYO nor its representatives will reproduce said photograph, interview or likeness for any commercial value or receive monetary gain for use of any reproduction/broadcast of said photograph or likeness outside of the marketing, public relations, and charitable purposes of CKYO. I also acknowledge that I (or student) will not receive monetary compensation for such media participation.

I hereby agree to release, indemnify, and hold harmless CKYO, its Board of Directors, employees, agents, volunteers or other representatives from any liabilities, known or unknown, and all damages of any kind arising out of the production or use of this material.

I certify that I have read the Media Consent and Release Liability statement and fully understand its terms and conditions.
