*Completing this form does not guarantee an appointment. All consultations are reviewed, and bookings are based on design fit, availability, and artist discretion.

*All clients must be 18 years or older with valid government-issued ID at the time of the appointment.

*A non-refundable deposit may be required to secure a booking. The deposit will go toward the final cost of your tattoo but may be forfeited in the event of a no-show or last-minute cancellation.

*By submitting this form, you agree that the tattoo design is a collaborative process. While your references are helpful, the final design will be created in the artist’s style.

*You acknowledge that you are in good health, not under the influence of drugs or alcohol, and have disclosed any medical conditions or allergies that could affect the tattoo process.

*All tattoo work is permanent and performed at your own risk. The artist is not responsible for healing outcomes once proper aftercare instructions have been provided.

⚫️Pricing- For large scale work i charge based off of an hourly rate of $175, which is typically applied to tattoos that take 3+ hours

⚫️However for smaller tattoos I charge by the piece as there can be many veriables involved, but i will add a sizing chart for smaller tattoos to help give you a general idea. Please keep in mind that the chart provided is just to help with estimates, and not always 100% accurate
