Waivers and Policies
* I am fully aware of and appreciate the risks, including the risk of catastrophic injury, paralysis, and even death, as well as other damages and losses associated with participation in dance activities and events. I further agree that any Bright Lights instructor and any staff at any Bright Lights event shall not be liable for any losses or damages occurring because of my child’s participation in the event, except where such loss or damage is the result of the intentional or reckless conduct of one of the organizations or individuals identified above. I grant permission for any instructor of Bright Lights Dance to take whatever steps they deem necessary to obtain emergency medical care for my student(s). I realize I am fully responsible for my dancer prior to and immediately following their class.
*My student(s) will only participate in Bright Lights Dance classes, competitions, and events for which I believe that they are physically and psychologically prepared. Prior to participation, they will have practiced their exercises and will perform only those exercises which they have accomplished to the degree of confidence necessary to assure they can perform them alone and without injury.
* I agree to pay the costs invoiced for the student(s) listed above to participate. Tuition, if not paid in advance, will be auto drafted on the first of the month or the closest business day and is the same each month regardless of absences or number of classes per month. I understand there will be no refunds given unless my child drops out of the program due to a serious medical condition or death of an immediate family member. I understand that even in these cases, some competition fees/costs may be nonrefundable.
*I hereby grant permission to Bright Lights Dance to post my and/or my child’s story, photos, videos, or other items, hereinafter referred to as “Materials,” to and for Bright Lights Dance social media, as well as any instructor’s social media as promotion for Bright Lights Dance Studio. I hereby release Bright Lights Dance, representatives, and instructors, from all claims/demands arising out of or in connection with any use of said “Materials” including, without limitation, all claims for invasion of privacy, infringement of my right of publicity, defamation and any other personal and/or property rights. I acknowledge and agree that no sums whatsoever will be due to me because of the use and/or exploitation of the “Materials” or any rights therein.
This notification of risk and enrollment has been read thoroughly, is understood completely and has been discussed with my dancer(s). It is being signed voluntarily, and I acknowledge its content and intent.
