I am requesting an exception for my raptor for the Certificate of Veterinary Inspection (health certificate) requirement for Utah. I agree that I will quarantine the bird away from other birds until test results are received and the bird is examined by a veterinarian.

I understand that I must test the bird for avian influenza within 10 days of arrival. I will request sampling information and a test kit from Dr. Van Wettere at the Utah Veterinary Diagnostic Lab at (435) 797-1895.
