Chic Massage Diva Spa Membership Agreement

This Membership Agreement (the "Agreement") is made between the individual named below ("Member") and Chic Massage LLC ("Spa"). By signing below, the Member agrees to the terms and conditions outlined herein.

Membership Term & Renewal

This membership is for an initial term of six (6) months and can only be cancelled according to the terms of this Agreement. After the initial period, the membership will continue on a month-to-month basis at a rate of $74. 

By enrolling, you commit to maintaining your membership for at least the initial six-month period. After this initial commitment, you may cancel at any time as described in the cancellation policy below. 

 

Payment Authorization

I authorize Chic Massage Diva Spa to charge my payment method monthly via PayPal subscription for the membership selected. I understand that PayPal will securely store and process my payment information, and that monthly charges will occur automatically until cancellation is done under the terms below.

Cancellation Policy

1. 15-Day Right to Cancel

You may cancel this agreement within fifteen (15) days of the Commencement Date for any reason. No refund will be issued. If you have already received a service during this period, you agree to pay the difference between the membership rate and the regular price of that service.

2. Cancellation After 15 Days (During Initial 6-Month Term)

Membership may only be canceled during the initial 6-month term under the following qualifying conditions:

• Death
• Disability, illness, or injury (doctor’s note required)
• Change in residence over 10 miles from the spa
• Unemployment or inability to continue payments
• If the spa fails to open, discontinues operations, or changes services substantially

3. Cancellation After Initial 6-Month Term

After your initial 6-month term, you may cancel your membership at any time by simply canceling your PayPal subscription. This will automatically stop future payments.

Chic Massage LLC will receive automatic notice from PayPal and will remove your name from the active membership roster accordingly.

Please note: if you have already received your monthly service and cancel afterward, you agree to pay the difference between the membership rate and the regular price of that service. No refunds will be issued for unused benefits.

4. Cancellation Month Usage

If you use a service during the cancellation month, you agree to pay the difference between the membership rate and the regular price for that service.

Terms & Conditions

1. Membership Fee

You are responsible for ensuring that a valid credit/debit card is on file via PayPal. If your payment fails, your membership will be paused after one missed billing cycle. If no updated payment method is provided within 10 days of the failed transaction, your membership may be suspended and/or terminated, and a $25 late fee may be applied.

You remain financially responsible for any missed payments or balances due during the period your membership is paused. Missed payments do not automatically cancel your agreement or remove your financial obligation.

2. Gift Cards and Promotions

Gift cards may not be used to pay for membership fees. The membership fee cannot be combined with other promotions or discounts.

3. Membership Transferability

Your membership is personal to you and may not be shared. However, as a member, you may purchase membership gift cards at $74 each for others to enjoy one month of services. 

• Are non-transferable once redeemed
• Must include recipient's name and contact information at time of purchase
• Cannot be resold or exchanged for cash

Chic Massage LLC reserves the right to deny redemption if misuse is suspected.

4. Loss of Property

Chic Massage LLC is not responsible for the loss, theft, or damage of personal items brought into the spa.

5. Contact Information

You agree to keep your contact and billing information up to date. Failure to do so may result in missed notifications and service interruptions.

6. Rules and Regulations

You agree to comply with spa rules. Chic Massage LLC reserves the right to suspend or cancel your membership for any conduct that endangers staff, clients, or the spa environment.

Membership Benefits

As a member, you are entitled to one (1) monthly service:

• 60-minute Swedish Massage
• 60-minute Regular Facial (Treatment facials may incur a $20-$50 copay)
• 60-minute Therapeutic or Deep Tissue Massage
• 10% off all additional treatments and products. (Service packages excluded)
• Complimentary sauna with every service
• Exclusive promotions and events for members

Upgrade Option: Add 30 minutes of massage for $35.

Members must book services in advance by appointment. For services not included in the list, members may pay the price difference; a copay may apply.

Monday – Friday: 9am – 8pm
Saturday: 9am – 5pm
Sunday: 9am – 3pm
