The renter/organization named above agrees to borrow or rent the church equipment listed on this form and will use it only for the stated purpose. Renter accepts full responsibility for any loss, theft, damage, or malfunction of equipment while in their possession. The church is not responsible for injury or damage caused by misuse or improper operation.

Renter agrees:
- Equipment will not be loaned or sub-leased to third parties.
- Equipment will be returned in the same condition received, by the agreed-upon return date.
- Any damaged, lost, or missing equipment will be replaced or repaired at the renter’s expense.
- Renter assumes liability for any injury, damage, or loss occurring during use.
