The driver named above agrees to operate church vehicles safely and responsibly, only for approved church-related purposes. The driver confirms they are 21 years or older, hold a valid driver’s license, and have provided proof of current insurance.

Driver agrees:
- Vehicle will be operated in compliance with all traffic laws.
- Vehicle will not be used to transport alcohol, illegal substances, or unauthorized passengers.
- Driver is responsible for all traffic tickets, tolls, and penalties incurred during use.
- Any accidents, damage, or mechanical issues must be reported to the church immediately.
- Driver assumes all liability for injury or damage arising from use of the vehicle.
- Vehicle must be returned with a full gas tank and clean interior.
Church insurance may not cover negligent, unauthorized, or personal use of the vehicle. Driver understands they may be personally liable in such cases.
