    Please read each section carefully as you complete the form.

    This semester, you can choose between two class days:

    • Wednesdays at Rancho San Rafael: 10:00–11:00 AM
    • Thursdays at Betsy Donnelly Park: 10:00–11:00 AM
  • Parent Participation & Program Details – Fall 2025 Little Sprouts

    By registering your child for the Little Sprouts program, you agree to actively participate in your child's class. If you are unable to participate, please contact the program director before completing this form.

    Parent participation includes:

    • Sitting with your child
    • Engaging in songs and conversations
    • Exploring and encouraging hands-on learning together
    • If a parent is not available, we welcome babysitters and/or other family members and/or anyone the primary caregiver gives permission to care for their child

    Program Schedule:

    • Runs from the first week of September through the week before Thanksgiving
    • Includes 11 one-hour classes
    • One week off for Fall Break

    Additional Activities:

    • Community events and field trips may be offered based on age-appropriate availability
    • Details will be provided in the Parent Handbook at the beginning of the semester

    Tuition:

    • $200 per semester (includes all materials and supplies; nonrefundable)
    • Sibling discounts: 20% off for the second child, 30% off for the third, and so on

  • Please register each participating child for their own spot in the class.

    If you plan to bring an older sibling, please register them as a mentor for that class (small discounted rate for mentors)

    Children under 18 months can attend alongside siblings as long as they are able to be in a carrier or stroller. We want those in this semester of Sprouts to get the most out of the class without distractions and have the full participation from their caregiver.

  • Photo Release:

    Beautiful Roots Education LLC., may desire to use a photo of your child captured during activities and group time. These photos may be used for promotional purposes including press releases, brochures, flyers, web postings, and social media advertising.

    By marking yes, you are agreeing to authorize Beautiful Roots Education LLC., to use your child’s photograph for promotional purposes.

    You acknowledge that only Beautiful Roots Education is authorized to use the image(s).

    You understand that you may revoke this authorization at any time, except to the extent that action based on this authorization has already been taken.

    You hereby release Beautiful Roots Education LLC., and its officers from any legal responsibility or liability for the disclosure of the images.

  • Professional Photo Release:

    Beautiful Roots Education LLC. is contracted with AB Photo Reno who may attend a group meeting or field trip at any time and desire to use a photo of your child captured during activities and group time. These photos may be used for promotional purposes including press releases, brochures, flyers, web postings, and social media advertising.

    By marking yes, you are agreeing to authorize Beautiful Roots Education LLC and AB Photo Reno to use your child’s photograph for promotional purposes.

    You acknowledge that only Beautiful Roots Education and AB Photo Reno authorization to use the image(s).

    You understand that you may revoke this authorization at any time, except to the extent that action based on this authorization has already been taken.

    You hereby release Beautiful Roots Education LLC and AB Photo Reno and its officers from any legal responsibility or liability for the disclosure of the images.

  • Liability and Waiver:

    I understand and acknowledge that Beautiful Roots Education LLC. is an outdoor-based program where children may participate in nature walks, climbing, hiking, and other activities involving natural terrain and materials, including but not limited to rocks, fallen branches, logs, and tree stumps.

    While Beautiful Roots Education takes great care in ensuring a safe environment—including CPR and First Aid training for its staff—there remains an inherent risk of minor injuries, such as trips, falls, or scrapes, due to the natural outdoor setting.

    We are committed to offering age-appropriate and safe outdoor experiences for all families.

    In addition, participants at locations such as, but not limited to, Caughlin Ranch, Callihan Park, and Rancho San Rafael Park, may encounter natural or man-made bodies of water, including streams, rivers, ponds, and creeks.

    While Beautiful Roots Education staff will make every reasonable effort to ensure the safety of all participants, it is ultimately the caregiver's responsibility to supervise their child near water at all times.

    By participating in this program, I voluntarily assume all risks associated with outdoor activities and agree to hold Beautiful Roots Education LLC, its owners, employees, and agents harmless from any claims, liabilities, or demands arising from any injury or incident.

    I waive my right to pursue legal action against the organization in the event of injury or accident and accept full responsibility for my child’s safety and well-being.
