This is an informed consent form for the Wheelie Master  bikeSimulator, a stationary motorcycle training device designed to help users safely practice balance and rear-wheel control in a padded and supervised environment.

1. PURPOSE AND FUNCTION

The Simulator allows riders to simulate a wheelie by adjusting the motorcycle’s angle from horizontal to vertical. This setup enables users to practice balancing at the rear-wheel balance point without riding on public roads. The motorcycle remains stationary and is equipped with safety padding and restraints.

2. AGE REQUIREMENTS
• Participants must be a 18 years old and above

3. USING YOUR OWN MOTORCYCLE
• A motorcycle is provided for use with the Simulator.
• If you wish to use your own motorcycle, you must:
• Present valid proof of insurance.
• Allow staff to take a temporary photo of your insurance for verification. This will be deleted after your session.
• By using your own motorcycle, you accept full responsibility for any damage that may occur:
1. While waiting,
2. While mounting or dismounting the Simulator,
3. During the simulation, or
4. In the event of malfunction or accident.

IMPORTANT: If you fail to provide valid insurance or present fraudulent documentation, your motorcycle will not be permitted on the Simulator.

4. RULES AND BEHAVIOR

We reserve the right to refuse or terminate participation for the following reasons:
• Aggressive or inappropriate behavior
• Refusal to follow safety instructions
• Presentation of fraudulent or invalid documents
• Any actions deemed unsafe by staff

5. SAFETY EQUIPMENT & INSTRUCTIONS
• All participants must follow all instructions given by staff.
• Participants may use their own protective equipment, at their own risk.
• Minimum required safety gear includes:
1. A helmet, and
2. Closed-toe shoes

Participants who do not wear minimum required safety gear will not be permitted to ride.

6. LIABILITY & ASSUMPTION OF RISK

By signing this form, you acknowledge and accept the following:
• There is an inherent risk of injury or equipment failure before, during, or after use.
• You may fall or be injured even with proper restraints, padding, and gear.
• We are not liable for:
• Injuries to you or others
• Damage to personal motorcycles or gear
• Theft or loss of personal items
• Simulator malfunction or breakage

7. FULL RELEASE OF LIABILITY

By signing this consent form, you agree to:
• Release the owners, operators, staff, and event hosts from any and all liability.
• Assume full responsibility for any injuries, losses, or damages incurred.
• Waive any right to bring legal action relating to the use of the Simulator, regardless of whether it is operated in a public or private setting.

8. ACKNOWLEDGEMENT

By participating in the Wheelie Master Bike Simulator, I confirm that:
• I have read and understood this entire consent form.
• I voluntarily accept all risks associated with the use of the Simulator.
• I agree to follow all instructions and wear all required safety gear.
• I understand that non-compliance will result in immediate removal without a refund.

NOTE: Use of the Simulator at any location (private or public) is entirely at your own risk. By participating, you waive the right to hold the owners, operators, or event organizers liable for any injury or damages that may occur before, during, or after Simulator use.
