  • I, the undersigned hereby acknowledge that my child is voluntarily participating in the Faith Over Fear Athletics Program. In consideration for being permitted to participate in the program, I agree to the following:


    Assumption of Risk: I am fully aware that participation in the program involves inherent risks, including but not limited to physical injury, illness, property damage, or other losses that may arise from my or my child's participation in program activities.

     

    Health and Medical Conditions: I confirm that my child is physically and mentally capable of participating in the program and have no known health or medical conditions that would prevent them from safely engaging in clinic activities. If there are any health or medical conditions that my child has, I have provided all necessary information to the program and will inform the program of any changes to this information.

     

    Release and Waiver of Liability: I hereby release, discharge, and hold harmless Faith Over Fear Athletics its organizers, employees, volunteers, and representatives from any and all claims, demands, actions, or causes of action arising out of or relating to my or my child's participation in the program, including but not limited to any negligence or breach of duty by the program or its staff.

     

    Indemnification: I agree to indemnify and hold harmless Faith Over Fear Athletics, its organizers, employees, volunteers, and representatives from any claims, damages, losses, or expenses, including reasonable attorneys' fees, arising out of or resulting from my or my child's participation in the program.

     

    Photographic and Video Release: I grant Faith Over Fear Athletics the irrevocable right and permission to use photographs and/or videos of me or my child taken during the program for promotional and marketing purposes.


    I have and without inducement read this Program Waiver and Release of Liability, fully understand its terms, and sign it voluntarily.

  • Attendance

    Athletes are expected to attend and be on time to every practice. Please notify staff of planned vacations prior to our season starting. We understand that emergencies arise and children will fall ill. However, once your athlete reaches 2 unexcused absences we may need to reevaluate their roles in the routine and/or placement on the team to ensure that the team is ready for competitions and that everyone is getting a full experience at the expected value.  


    Athlete Expectations
    Athletes will give their best effort at all times. Athletes will not swear or use abusive language at any time. Athletes will not belittle or laugh at others because of experience, inability or mistakes. Athletes are required to practice in cheer or gym shoes. Athletes will use self-discipline and work to respect each other's feelings. Athletes will work together as a team to promote sportsmanship. Athletes will respect each other and their coaches at all times. If behavior issues arise, parents will be notified immediately. If behaviors are consistent and are disruptive to the team, the athlete will be removed from the program.

    We are a "No Drama" organization. We pride ourselves on operating in a healthy family dynamic. All conflicts/issues should be reported immediately to the director or coaches and will be handled accordingly.

     

    Tuition Policy

     Due Date and Grace Period:
    - Tuition payments are due on the 1st of each month.
    - A grace period of 5 days is allowed after the due date to submit payment without penalty.

    Tuition Payment Dates:

    Practice Wear Deadline: September 1, 2025
    Registration Deadline: September 27, 2025
    Uniform Deadline: October 1, 2025
    October Tuition: October 1, 2025
    Custom Cheer Shoe Deadline: October 6, 2025
    November Tuition: November 1, 2025
    December Tuition: December 1, 2025
    January Tuition: January 1, 2026
    February Tuition: February 1, 2026
    March Tuition: March 1, 2026
    April Tuition: April 1, 2026
    May Tuition: May 1, 2026

    Please Note: Payments can be made at any time prior to the listed due dates. However, all payments are non-refundable.

    Late Payment Protocols:

    - First Instance: If payment is not received by the end of the 5-day grace period, a reminder notice will be sent via email or other agreed-upon communication method.

    - Second Instance: If payment remains outstanding 10 days after the due date, a late fee of $35 will be applied to the account.


    - Third Instance: Should payment not be received within 15 days of the due date, the athlete may be suspended from participation in practices and events until the account is brought up to date.


    - Final Instance: In cases where payment is severely overdue (more than 30 days past due), the athlete may be withdrawn from the program, and their place may be offered to another participant on the waiting list.

    Communication and Support:
    - It is the responsibility of the athlete's guardian to ensure timely payment of tuition.
    - Any issues or difficulties regarding payment should be communicated promptly to the Gym Director or Gym Coordinator to discuss possible solutions or arrangements.

    Reinstatement and Re-enrollment:
    - Athletes who have been suspended due to overdue tuition may be reinstated upon full payment of outstanding balances and any applicable late fees.
    - Re-enrollment after withdrawal due to non-payment will depend on availability and at the discretion of the program director.

    By adhering to this policy, we ensure smooth operation of our program and maintain fairness to all participants. Thank you for your cooperation in supporting our cheerleading community.

     

    Parent Code of Conduct Agreement


    At Faith Over Fear Athletics, we are committed to creating a positive, empowering, and respectful environment for all athletes, coaches, staff, and families. As a parent or guardian, your partnership is vital in upholding the values of our program.

    By signing below, you agree to the following expectations:


    1. Positive Support & Encouragement


    I will encourage my athlete to do their best, maintain a positive attitude, and enjoy their cheer experience.


    I will support all athletes, not just my own, and applaud effort, not just results.


    I will refrain from coaching my child or other athletes from the sidelines during practice or competitions.


    2. Respect for Coaches & Staff


    I will respect the authority of coaches and staff and trust their decisions regarding placements, routines, and disciplinary actions.


    I understand that questioning or challenging decisions in a disrespectful manner is not appropriate and may result in disciplinary action, including possible removal from the program.


    I will follow proper communication channels and allow 24 hours before discussing concerns related to emotional matters.


    3. Attendance & Commitment


    I will ensure my athlete attends all required practices, events, and competitions unless excused for illness, injury, or emergency, and will communicate any conflicts in advance.


    I understand that excessive absences may affect my child’s role in routines or team placement.


    4. Financial Responsibility


    I agree to make timely payments as outlined in the program’s financial policies.


    I understand that missed payments may result in my athlete being withheld from practices, events, or performances.


    I acknowledge that all payments are non-refundable.


    5. Program Integrity & Conduct


    I will not engage in gossip, drama, or negative talk about athletes, parents, coaches, or staff, whether in person or online.


    I will refrain from using profanity or aggressive behavior at any Faith Over Fear event or practice.


    I understand that poor behavior, including on social media, may lead to disciplinary action or removal from the program.


    6. Communication & Engagement


    I will stay informed by checking all official communications and group platforms regularly.


    I will communicate with staff respectfully and in a timely manner and respect boundaries, including outside of business hours unless there is an emergency.


    I will support my child’s team by attending events, being on time, and volunteering when able.

    I have read and agree to all the information on this form and provided to me on the Parent/Athlete informational packet. I understand that failure to comply to these rules may result in removal from the team. By signing up for Faith Over Fear Athletics I am agreeing to commit to the team from October 2025 - May 30, 2026. I understand that if I/my athlete decides not to continue with the team after beginning the season I will forfeit all monies paid and will owe a $200 breach of commitment fee.
