    Submitting this application does not automatically enroll you in the program or guarantee acceptance. Please answer all questions accurately. If your application is approved, you will receive additional information regarding program requirements, required consumer disclosures, agreements, credit monitoring and onboarding.

  • Section 3 — Required Credit Monitoring

  • Our program requires active three-bureau credit monitoring for the duration of services. The monitoring service is provided by a separate third-party company and currently costs approximately $29.99 per month. Are you willing and financially able to maintain the required credit monitoring service while participating in the program?*
