• Have you been placed on a Performance Improvement Plan (PIP)?

    A PIP is a formal document outlining concerns about a resident’s performance, with specific goals and a timeline for improvement. While it is intended to support remediation, it can also precede termination — even though that is not its intended purpose.

    The Housestaff Association is collecting optional, confidential reports from union members to:

    • Ensure all appropriate remediation efforts have been pursued before any steps toward termination,
    • Confirm that Cook County Health is following the negotiated terms of the Housestaff Collective Bargaining Agreement — particularly regarding evaluations, their timing, and the required advising and remediation process.

    This form is entirely optional and confidential.
    However, in the event of a discharge or termination, it is extremely helpful for the union to have this documentation in advance to better support and advocate for the affected resident.
