Private Lesson Registration Form
A registration form is required for ALL students seeking private lessons from the Lawrenceburg Academy of Performing Arts. Completion of this form and payment of a $30 registration fee guarantees your student a spot on our waiting list and/or a scheduled lesson day/time when openings become available. A registration form is required for EACH child you are registering for private lessons.
