Welcome to the Athlete Intake form! Im happy to have you here and excited to completely transform you lifestyle, body, and psyche! Please take a moment to read all the questions thoroughly and reply with as much information as possible. This helps me understand you as a person. 

The rate for my athlete coaching is a flat rate of $350, automatically billed monthly. This gets you unlimited 24/7 access, weekly check ins (with a full detailed breakdown sent back), exercise review, posing critiques (If Applicable), a fully custom, tailored to you, exercise, nutrition, supplementation and PED protocol (If applicable). Plus a fully custom peak week/showtime protocol (If applicable). This is a full package, there is nothing you need to worry about beyond this, every base is covered.

By submitting this form and participating in coaching services provided by Reese Rutko/Precision Coaching Systems, you acknowledge and agree to the following:

6. Waiver of Liability: You release Reese Rutko/Precision Coaching Systems from any liability related to injuries, health complications, or outcomes that may occur as part of this coaching program.
7. Consent: You understand and voluntarily accept all terms listed in this disclaimer. Please note, upon finishing coaching, the last payment will be taken out.
