All personal information you provide is collected for the purpose of assessing whether Ethics of Care is the right service to support your therapeutic needs.
The information you submit through this form is stored securely in our encrypted electronic database and is accessed only by authorised personnel involved in your care.
Your information will be retained only until the completion of your initial assessment. Once the assessment is concluded, your personal information will be permanently deleted from our systems, and no identifiable information will be kept on file.
We are committed to protecting your privacy in accordance with the Privacy Act 1988 (Cth) and the Australian Privacy Principles. By submitting this form, you consent to the temporary storage and use of your information for the stated purpose.
