LavaQuest Birthday Party Participant Agreement and Waiver
Liability Waiver
Participating in a LavaQuest birthday party involves certain risks that could result in physical or emotional injury, including but not limited to slips, falls, collisions, and other accidents that may cause bruises, sprains, fractures, or more serious injuries.
By choosing to celebrate your birthday with LavaQuest, operated by Lava Quest LLC in South Carolina, you acknowledge and agree to the following:
Assumption of Risks: I understand that participating in activities at LavaQuest carries inherent risks, both known and unknown. I accept full responsibility for any injuries or damages that may occur.
Release of Liability: I release LavaQuest, its owners, staff, affiliates, and volunteers from any claims or liabilities arising from my participation, including those caused by negligence.
Rules and Conduct:
I agree to follow all safety instructions and rules provided by LavaQuest staff.
I understand that failure to follow these rules may result in removal from the premises without a refund.
Medical Authorization: In case of an emergency, I authorize LavaQuest staff to seek medical treatment for myself or the minor(s) in my party and agree to pay any related costs.
Health Declaration: I confirm that I and my party are in good health and do not have conditions that would prevent safe participation. We agree not to partake while under the influence of alcohol, drugs, or medications affecting our abilities.
Insurance: I confirm that I have adequate insurance coverage for any injuries or damages incurred during the party or I agree to cover such costs myself.
Legal and Media:
This agreement is governed by South Carolina law.
I understand that if any part of this waiver is unenforceable, the rest remains valid.
I consent to LavaQuest using photos or videos taken during the party for promotional purposes, including online sharing, without compensation.
By signing below, I acknowledge that I have read and understand this waiver and agree to be bound by its terms. I recognize that signing this document waives certain legal rights.
