Consent & Liability Waiver

Port City Youth Football League participants must acknowledge this Consent Form & Liability Waiver to be to participate in activities. I hereby grant permission for my team to participate in various training and sports activities (hereinafter referred to as activities) operated or sponsored by Port City Youth Football League and it's subsidiaries.

I understand and acknowledge that a participant may incur personal or bodily harm while participating in sports-related activities, including but not limited to activities described as running, jumping, physical contact, etc. By acknowledging this agreement, I assume all risks inherent in these activities and accept full responsibility for any and all damages or injuries of any kind; but I am also aware that reasonable care and supervision will be exercised by the adult supervision to provide for the general well-being of my team.

I understand and acknowledge that participation also includes possible exposure to and illness from infectious diseases including but not limited to MRSA, influenza, and COVID-19. While particular rules and personal discipline may reduce this risk, the risk of serious illness and death remains. I, on behalf of myself/the minor registrants, knowingly and freely also assume all such exposure and illness risks, both known and unknown, EVEN IF arising from the negligence of the Released Parties or others, and assume full responsibility for my/the minor registrant’s participation.

I, on behalf of myself/the minor registrants, agree to abide/ensure that the minor registrant abides by all rules and policies of the Club, the City, and the facility operators, including compliance with the terms and conditions for participation related to infectious diseases.

I understand and assume the risks described above and those inherent with such activities, and I individually, on behalf of my child, do hereby release and discharge and covenant not to sue Port City Youth Football League, its officers, directors, employee's agents and/or volunteers for this activity and agree to hold them harmless from any and all claims whatsoever arising from the conduct of any person(s) which results in the injury or loss to my child or myself. I accept responsibility of notifying the League Director or training staff of any pre-existing conditions affecting my team members.

I further understand and acknowledge that Port City Youth Football League will not allow my team to participate in such activities without my acceptance of and signature on this agreement.

This Release, Hold Harmless and Waiver agreement shall be construed broadly to provide a release, hold harmless and waiver to the Released Parties to maximum extent permissible under applicable law. I understand that this is a contract and certify by acknowledging that I have read this document and have no questions thereon.
