Authorization to Collect Pet Travel Information

By signing below, I authorize Pet Health Certificate Guru and its representatives to collect, store, and use information necessary to assist in the preparation, planning, and documentation related to my pet’s domestic or international travel. This includes, but is not limited to:

Health records, vaccination history, and veterinary contact information

Microchip and identification details

Travel itineraries, destination requirements, and transportation preferences

Communication with veterinary clinics, USDA offices, and other regulatory or service entities (with my additional consent as needed)

I understand that the information collected will be used exclusively to:

Assist in preparing travel documentation (including health certificates and endorsements)

Provide regulatory guidance and travel readiness support

Ensure compliance with airline, state, and/or international requirements
