To request a Carer Card we require you to provide the following information before we can process your application:

If you are a carer and you are not already known to Lanarkshire Carers, we cannot issue you with a Carer Card and your application will not be processed. To check if you are known to us, please contact Lanarkshire Carers Centre: Hamilton on 01698 428090 or Lanarkshire Carers Centre: Airdrie on 01236 755550.

Alternatively, please complete our Online Self-Referral Form indicating Carer Card as a reason for referral and a member of staff will pick up with you in due course.

You are applying for our standard Digital Carer Card, which will be conveniently available for download to a wallet app on your smartphone.

Effective January 1st 2025, Lanarkshire Carers have discontinued mailing physical cards. Whilst physical cards can still be requested, they must be collected in person from one of our two centres. You will be notified by email/phone call once your card is ready for pickup.

Lanarkshire Carers Centre: Airdrie -- 92 Hallcraig Street, Airdrie, ML6 6AW

Alternatively, you may opt for a digital card, which will be emailed to you and can be conveniently added to a mobile wallet app on your smartphone.

Please be advised that any physical cards not collected within 30 days of collection notification will be securely destroyed in compliance with GDPR regulations. In such cases, a digital card will be offered as an alternative.

Photo requirements

The photo you supply for your Carer Card should be recent, front-facing and centred, showing your face clearly and without any filters applied. The size of your photo must not exceed 2MB.

You can either upload a suitable existing digital photo of yourself or use the 'Take Photo' feature on this form to capture a new one.

If the photo is not suitable, we may not be able to progress your application. If you experience any issues with this, please contact us on 01698 428090.

You have chosen to visit one of our centres to progress your carer card application.

You do not need to do anything at the moment. When you visit one of our centres, a member of staff will support you in taking your photo and with submitting the completed form at that time.

You are welcome to visit either Lanarkshire Carers Centre: Hamilton or Lanarkshire Carers Centre: Airdrie, Monday - Friday 9:00 AM to 4:30 PM and our staff will be happy to help. You do not need an appointment for this.
